The aim of our paper is to define the patient that has four cardiac problems and repaired all of them in a single procedure. 12 year old boy has 1- subvalvular discrete membrane, 2- bicuspid aortic valvular stenosis, 3- supravalvular aortic stenosis and 4- patent ductus arteriosus.
