It is therefore essential to know the overall cardiovascular risk and reveal the predictors of metabolic control in Turkish adults with diabetes mellitus. Methods: A nationwide, multicenter survey consecutively enrolled patients who were under follow up for at least a year. Optimal control was defined as HbA1c < 7%, home arterial blood pressure (ABP) < 135/85 mmHg, or LDL-C < 100 mg/dL. Achieving all parameters indicated triple metabolic control. Results: HbA1c levels of patients (n = 5211) were 8.6 ± 1.9% (71 ± 22 mmol/mol) and 7.7 ± 1.7% (61 ± 19 mmol/mol), in Type 1 and Type 2 diabetes, respectively. Glycemic control was achieved in 15.3% and 40.2%, and triple metabolic control was achieved in 5.5% and 10.1%, respectively. Only 1.5% of patients met all the criteria of being non-obese, non-smoker, exercising, and under triple metabolic control. Low education level was a significant predictor of poor glycemic control in both groups. Conclusions: Few patients with Type 2, and even fewer with Type 1 diabetes have optimal metabolic control in Turkey. The LDL-C target attainment and the characteristics of patients attaining these targets were investigated in this study. Furthermore, the reasons for not choosing statins and the physicians’ attitudes on the treatment of diabetic dyslipidemia were also examined. Methods: A nationwide, cross-sectional survey was conducted in tertiary centers for diabetes management. Adult patients with T2DM, who were under follow-up for at least a year in outpatient clinics, were consecutively enrolled for the study. LDL-C goals were defined as below 70 mg/dL for patients with macrovascular complications or diabetic nephropathy, and below 100 mg/dL for other patients. Data about lipid-lowering medications were self-reported. Results: A total of 4504 patients (female: 58.6%) were enrolled for the study. The mean HbA1c and diabetes duration was 7.73 ± 1.74% and 10.9 ± 7.5 years, respectively. The need for statin treatment was 94.9% (n = 4262); however, only 42.4% (n = 1807) of these patients were under treatment, and only 24.8% (n = 448) of these patients achieved LDL-C targets. The main reason for statin discontinuation was negative media coverage (87.5%), while only a minority of patients (12.5%) mentioned side effects. Physicians initiated lipid-lowering therapy in only 20.3% of patients with high LDL-C levels. It was observed that the female gender was a significant independent predictor of not attaining LDL-C goals (OR: 0.70, 95% CI: 0.59–0.83). Conclusions: Less than 50 % of patients with T2DM who need statins were under treatment, and only a quarter of them attained their LDL-C targets. There exists a significant gap between the guideline recommendations and the real-world evidence in the treatment of dyslipidemia in T2DM. © 2020, The Author(s).Item An evaluation of subsidies granted to the private educational institutions within the framework of Turkish tax system(Peter Lang AG, 2020) Özer A.Ö.; Özer B.; Akin S.One of the essential concerns that governments have to deal with has been the theme of economic growth and development. In order to be able to sustain the objectives mentioned above, states and governments instrumentalize fiscal policies in which subsidies occupy a substantial place. The primary rationale behind these fiscal policies, including grants, has been the allocation of resources to those fields with better and more efficient prospects within the general good of the economy. Despite convergences seen in terms of types and implementation of subsidies, the basic objective is to accomplish higher rates of economic growth and investment. Through the Decision of the Council of Ministers of the Turkish Republic dated June 19th, 2012, investments to be handled for primary, secondary, and high school educational institutions were evaluated within the framework of the fifth region with the labeling of priority investment. Along with the closure of private-mentoring facilities, the related facilities investors were foreseen to utilize the subsidies to convert these facilities to schools, thereby minimizing the costs of investments coupled with rises in investments. Changes in the whole body that occur with aging also affect swallowing functions and cause presbyphagia. This condition may progress to oro-pharyngeal and/or esophageal dysphagia in the presence of secondary causes that increase in incidence with aging. However, no study has been published that provides recommendations for use in clinical practice that addresses in detail all aspects of the management of dysphagia in geriatric individuals. This study aimed to answer almost all potential questions and problems in the management of geriatric dysphagia in clinical practice. Methods: A multidisciplinary team created this recommendation guide using the seven-step and three-round modified Delphi method via e-mail. The study included 39 experts from 29 centers in 14 cities. Results: Based on the 5W and 1H method, we developed 216 detailed recommendations for older adults from the per-spective of different disciplines dealing with older people. Conclusion: This consensus-based recommendation is a useful guide to address practical clinical questions in the diagnosis, rehabilita-tion, and follow-up for the management of geriatric dysphagia and also contains detailed com-mentary on these issues. © 2022 by The Korean Geriatrics Society.Item The Asymmetric Effect Of Real Effective Exchange Rate Volatility On Agricultural Foreign Trade: The Example Of Turkey In The Period 2013-2021; [Reel Efektif Döviz Kuru Oynaklığının Tarımsal Dış Ticaret Üzerindeki Asimetrik Etkisi: 2013-2021 Arası Dönemde Türkiye Örneği](Turkish Journal of Agricultural Economics, 2022) Akin S.; Duramaz S.; Koçtürk O.M.Purpose: The recent extreme volatilities in exchange rates have made it important to examine exchange rate volatility on foreign trade. The aim of this study is to reveal the asymmetric effect of real effective exchange rate volatility on agricultural foreign trade in terms of different economic conjunctures in the period 2013:01-2021:08. Design/Methodology/Approach: ARMA(1,0)-EGARCH(1,1) model was used to measure exchange rate volatility. The effect of exchange rate volatility on the foreign trade effect was analyzed with the MSIAH model. Findings: According to the results, it has been revealed that the effect of real exchange rate volatility on foreign trade differs in terms of economic expansion and contraction regimes, and real exchange rate volatility in the economic expansion regime has an increasing effect on imports. Originality/Value: The study is expected to draw more attention to agricultural problems in the recent period, and to contribute to the solution of problems related to agricultural foreign trade, especially in the exchange rate axis. © 2022, Turkish Journal of Agricultural Economics.
