Item A descriptive study: Psychiatric diseases accompanying obstructive sleep apnea syndrome (OSAS); [Tanimlayici bir çalişma: Obstrüktif uyku apne sendromuna (OSAS) eşlik eden psikiyatrik hastaliklar](2008) Bakar E.E.; Yilmaz H.Purpose: OSAS is a disease which involves a wide range of symptoms from simple snoring extending up to the tables accompanied by serious cardiovascular, pulmonary, endocrine and psychiatric diseases. In our study; we aimed to look into the variety of the psychiatric diseases observed in patients with diagnosis of OSAS and to discuss the spectrum of psychiatric diseases accompanied by the typical clinical symptoms and findings in light of literature. Methods: The cases who visited our outpatient clinic in between 2004-2007 and reveived preliminary diagnosis of OSAS were inspected all night through video-EEG-polysomnography by certificated sleep technicians using devices of Embla A-10, N-7000Ë in accordance with sleep apnea protocol in order to trace the change in sleep structure and breathing events. All the cases were assessed by a psychiatric expert and those who had the diagnosis of psychiatric disease according to the criteria of DSM IV were included in the study. Results: Of 235 cases, in 23.8%, there found to have at least one comorbid psychiatric disease. In these cases, the frequency of the depression was 92.8%, schizophrenia was 7.1%, OCD (obsessive compulsive disorder) was 3.5%, and bipolar disorder - depressive episode was 1.7%. OSAS and the sleep structure of the cases having psychiatric disorders were investigated. AHI (Apnea-hypopnea index) increased in 68% of the cases. Discussion: The sleep continuity in OSAS gets broken and is fragmental. The superficial sleep duration increases, deep sleep and REM sleep get suppressed. In such psychiatric diseases as depression difficulty in falling asleep, destruction in sleep integrity due to the frequent wake-ups are observed. In generalized anxiety disorder and schizophrenia, a fragmental sleep and decrease in sleep efficiency are observed. When the psychiatric disease is accompanied by OSAS, high rate of AHI are frequently encountered. Conclusion: Our study examines the variety of the psychiatric diseases observed in patients with diagnosis of OSAS and makes discussions in light of the psychiatric disease spectrum literature data accompanying the OSAS clinic. Similar studies in bigger serials will ensure the psychiatric disease spectrum seen in cases with OSAS to be understood and diversities in approach against these cases to get clarified.Item A rare cause of late dystonia: Metachromatic leukodystrophy with adulthood onset (case report); [Nadir görülen bir geç distoni nedeni: Erişkin başlangıçlı metakromatik lökodistrofi (olgu sunumu)](Gulhane Askeri Tip Akademisi, 2009) Selçuki D.; Bakar E.E.; Kömürcülü N.Metachromatic leukodystrophy is an autosomal recessively inherited disease, which is characterized by deficiency of arylsulfatase A enzyme and diffuse symmetrical demyelinization in cerebral white matter. A 38-yearold male patient admitted with the complaints of generalized tonic clonic seizures and common involuntary contractions in the whole body, which both had started 8 years ago. Generalized dystonia was detected in the patient in whom walking and speech disturbances additionally began 3-4 months ago. Diffuse subcortical, periventricular homogenous hyperintense lesions were detected in T2-weighted axial and coronal scans in cranial magnetic resonance imaging. The diagnosis of metachromatic leukodystrophy was established with all these findings. Metachromatic leukodystrophy with adulthood onset may present with seizures and changes in personality. We would like to emphasize that metachromatic leukodystrophy, one of the metabolic diseases, may be encountered as a cause of adulthood onset dystonia by making a literature review about the diagnosis, clinical presentation and treatment choices of the disease in this paper. © Gülhane Askeri Ti{dotless}p Akademisi 2009.Item Neurosarcoidosis: Report of a case with pituitary-hypothalamic involvement; [Nörosarkoidoz: Hipofiziyer-hipotalamik tutulumlu bir olgu sunumu](Gulhane Askeri Tip Akademisi, 2009) Bakar E.E.; Selçuki D.A 38-year-old female patient admitted to our clinics with headache and itching in whole body. In cranial magnetic resonance imaging performed for the investigation of headache, hypothalamic mass lesion and leptomeningeal sarcoid foci were detected. The diagnosis of neurosarcoidosis was made on the basis of clinical and laboratory findings and radiological imaging. Cases with sarcoid foci in the form of hypothalamic mass lesion are very rare in the medical literature. TTH worsens quality of life and is related to various psychosocial factors. We aimed to examine the severity of headache (intensity, frequency, and duration), analgesic use, quality of life (QoL), and the impact of COVID pandemic-induced stress in TTH patients. Methods: TTH cases seen at neurology outpatient clinics in 15 centers in Turkey were included in the study. A questionnaire incorporating sociodemographic and medical information, headache features, sleep quality, general quality of life, and impact of the pandemic event was administered to the subjects. Results: A total of 975 TTH patients were evaluated. Headache severity was higher in women as well as in patients with a history of COVID-19 contact. Women, those with chronic diseases, and cases with a COVID-19 contact history had worse perceptions of quality of life and were affected to a greater extent by the pandemic. The factors affecting the impact of the pandemic were female gender and difficulty in access to health services for headache. Co-existing chronic diseases and lost productive time due to headaches were negative determinants for both QoL and the impact of the pandemic. Conclusion: Our results show that the COVID pandemic severely worsened the headache burden, quality of life and mental health of TTH patients. These findings can guide us in the clinical approach to TTH cases. © 2022, ASEAN Neurological Association. All rights reserved.
