    Subfoveal perfluoocarbon liquid retention after retina detachment surgery; [Retina Dekolman Cerrahisi Sonrasi Subfoveal Perflorokarbon]

    This study presents two cases that had subfoveal perfluorocarbon liquid (PFCL) after rhegmatogenous retinal detachment surgery. The first case was a 65-year-old pseudophakic male patient. The superior temporal retinal tear and a bullous retinal detachment that reached the macula border was observed in the right eye. The patient underwent 23 G pars plana vitrectomy (PPV). Intravitreal perfluorocarbon liquid was used to reattach the retina and perform laser retinopexy around the tear. The surgical procedure was ended by PFCL-air and air-silicone exchange. In the first month, subfoveal PFCL droplets were observed in the control examination. The patient, who was reoperated, was injected with an infusion liquid using a 38 G cannula from the perifoveal area; liquid and decalin droplets were re-aspired using a flute cannula. There were no PFCL droplets in the control examination. Visual acuity, which was 0.2 pre-operatively, increased to 0.3 post-operatively, but the patient complained of metamorphopsia . The second case was a 66-year-old male patient. There was a retinal tear in the right eye at the 5 o'clock position, and a retinal detachment in the lower half. The patient underwent a 23 G PPV operation. Intravitreal perfluorocarbon liquid was used to reattach the retina. First silicone and then heavy silicone oil was used in two subsequent surgeries. In the final control examination performed after silicone removal, PFCL was detected in the subfoveal area. The patient was followed up without surgery, had a visual acuity of 0.2, and a central scotoma was present in the superior temporal fixation center of the visual field.

    Introduction: There is very few data on the epidemiological features of interstitial lung diseases (ILD) in the literature. These studies on this subject suffer from limited number of patients. Objective: The goal of this study was to evaluate the epidemiological features of ILD in Turkey. Methods: Fifty-four investigators, 31 centres in 19 cities from six regions of Turkey, participated in the study. Two thousand two hundred forty-five newly diagnosed patients (51.8% females), led by Turkish Thoracic Society Clinical Problems Study Group, enrolled in this prospective study. Results: The mean age was 51.8±16.7 years. The mean age among males was 50.5±18.6 years and 53.0±14.6 years among females (P<0.001). 23.8% of the cases had ILD with known causes, while 39.4% were in granulomatous group, 23.7% were idiopathic, and 4.4% were in the unclassified group. Overall, histopathologically confirmed diagnosis rate was 40.4%. Sarcoidosis was the most common disease (37%), whereas cases with idiopathic pulmonary fibrosis (IPF) constituted 19,9% of patients. 53% of the sarcoidosis patients were females, and the ratio reaches to 75% under 50 years of age (for this group, IPF ratio is %3). In contrast, sarcoidosis and IPF ratios were equal in males (25%). Sarcoidosis was 8% in men over 50, while IPF was %45. Conclusion: The overall incidence of ILD in Turkey was computed to be 25.8/100000. © 2013 John Wiley & Sons Ltd.

    Evaluation of subclinical multiple sclerosis patients using pattern visual evoked potentials and visual field test; [Subklinik multipl skleroz hastalari{dotless}ni{dotless}n desen görsel uyari{dotless}lmi{dotless}ş potansiyeller ve görme alani{dotless} testi ile deǧerlendirilmesi]

    Objectives: The aim of this study was to evaluate subclinical multiple sclerosis (MS) patients with normal visual acuity using pattern visual evoked potentials (PVEP) test and central 30-2 threshold visual feld (VF) test. Materials and Methods: A total of 60 patients, 30 of which had a definitive MS diagnosis and 30 who were healthy volunteers, were included in the study. The participants were divided into the following three groups: Group 1 - optic neuritis (+) MS patients, Group 2 - optic neuritis (-) MS patients, and Group 3 - control group. Using white on white perimetry, SITA FAST central 30-2 threshold VF tests and 1°-0.3° PVEP measurements were performed. MD, PSD, p100 latency (ms), p100 amplitude (μV), and central 0-10°, 0-20°, 0-30°, 10-20°, 10-30°, 20-30° retina sensitivities (db) were evaluated in all groups. Results: There was a significant difference in all parameters between Group 1 and Group 3 (p<0.05). There was a significant difference in p100 latency 1°- 0.3°, p100 amplitude 0.3°, central 0-30°, 10-30°, 20-30° between Group 2 and Group 3 (p<0.05). There was no significant difference in the parameters except for p100 amplitude 1° between Group 1 and Group 2 (p>0.05). Conclusion: The evaluation of PVEP and central 30° VF can be used to monitor disease prognosis in subclinical MS patients.

    Septo-optic dysplasia: A case report; [Septo-optik displazi: Olgu sunumu]

    An 18-year-old male patient was admitted with loss of vision in the right eye and blurred vision in the left eye. The right eye did not respond to the stimuli in the pattern visual evoked potentials (PVEP) test, and there was a signifcant delay in the p100 latency in the left eye. Cranial magnetic resonance imaging (MRI) showed a decrease in the calibration of both optical nerves, absence of septum pellucidum, and deformity in both frontal horns of the side ventricles. Despite the rare incidence of Septo-optik displazi (SOD), it should be considered in cases of unilateral or bilateral visual complaints, especially if optical nerve hypoplasia is also present.

    Pyogenic granuloma is a benign proliferative vascular tumor that may be seen at areas of intense vascularization due to chronic inflammatory reaction. It also can be seen due to surgical trauma and materials used during operation. For treatment, removal of surgical material and surgical excision of granuloma are mostly sufficient. In this paper, a patient with conjunctival granuloma following combined classical retinal detachment surgery and pars plana vitrectomy is presented. © 2015 Gazi Eye Foundation. All Rights Reserved.

    Sleep disorders constitute an important public health problem. Prevalence of sleep disorders in Turkish adult population was investigated in a nationwide representative sample of 5021 Turkish adults (2598 women and 2423 men, response rate: 91%) by an interviewer-administered questionnaire. Insomnia was defined by the DSM-IV criteria, habitual snoring and risk for sleep-related breathing disorders (SDB) by the Berlin questionnaire, excessive daytime sleepiness (EDS) by the Epworth sleepiness scale score, and restless legs syndrome (RLS) by the complaints according to the International Restless Legs Syndrome Study Group criteria. Mean age of the participants was 40.7 ± 15.1 (range 18 to 90) years. Prevalence rates (men/women) were insomnia 15.3% (10.5%/20.2%; P < 0.001), high probability of SDB 13.7% (11.1%/20.2%; P < 0.001), EDS 5.4% (5.0%/5.7%; P: 0.09), RLS 5.2% (3.0%/7.3%; P < 0.001). Aging and female gender were associated with higher prevalence of sleep disorders except for habitual snoring. Prevalence rates of the sleep disorders among Turkish adults based on the widely used questionnaires were close to the lower end of the previous estimates reported from different parts of the world. These findings would help for the assessment of the health burden of sleep disorders and addressing the risk groups for planning and implementation of health care.     Chandelier retroillumination-assisted cataract surgery during vitrectomy

    Purpose To present an alternative cataract surgical technique in patients with cataract and intravitreal hemorrhage.MethodsTwelve cases with poor fundus reflex caused by severe vitreus hemorrhage were included in the study. All patients underwent combined phaco surgery and 23-gauge vitrectomy. Chandelier retroillumination was inserted into the infusion trochar during the cataract operations and was used with low power (50-75%) in necessary steps.ResultsWe did not experience phaco complications or complications due to technique during surgery. Retroillumination assistance was especially useful during final stages of cataract surgery, particularly irrigation-aspiration of cortical material.ConclusionRetroillumination in absence of red fundus reflex may be helpful and can be practiced more often in cataract surgery combined with vitrectomy.

    Review on the Eyedrop Self-Instillation Techniques and Factors Affecting These Techniques in Glaucoma Patients

    Objective. This study aims to evaluate eyedrop self-installation techniques and factors affecting these techniques in glaucoma patients. Methods. Researchers directly observed eyedrop instillation procedures of 66 glaucoma patients. Contact with periocular tissues and instillation onto ocular surface or conjunctival fornices were considered. Correlations of instillation patterns with patient characteristics including age, gender, intraocular pressure, cup-to-disc ratio, visual field loss, and total intake of glaucoma medication and handgrip strength score were searched. Results. The average handgrip strength in the instillation without periocular contact group was 66.4 ± 19.7 kg, while the average handgrip strength score was 55.9 ± 20.9 kg in the instillation with contact group. The difference between the two groups was statistically significant (p = 0.039). No statistically significant correlation was found between handgrip strength and the mean number of glaucoma medications, c/d, intraocular pressure (p > 0.05). Also there was no significant relation between mean handgrip strength score and the severity of the visual field defect (p = 0.191). Conclusion. Patients especially with severe glaucomatous damage should be adequately instructed about the proper techniques for self-instillation of eyedrops and motivated to use a proper technique. Also, it is possible to suggest that patients with a higher handgrip strength, indicating the well-being of general health, may be doing better in properly instilling glaucoma eyedrops. © 2016 Ozcan Kayikcioglu et al.

    The evaluation of scleral buckling and pars plana vitrectomy in the management of primary rhegmatogenous retinal detachment

    Purpose: To evaluate the results of scleral buckling (SB) and pars plana vitrectomy (PPV) surgeries for the treatment of rhegmatogenous retinal detachment. Materials and Methods: This study included cases who received surgical treatment due to primary rhegmatogenous retinal detachment in our clinic, between March 2003 and April 2014. According to surgery techniques, the cases were evaluated retrospectively and divided into two groups as SB and PPV. The groups were evaluated for anatomical and functional success and complications. Results: Single-surgery anatomic success was achieved in 48 of 52 (94%) eyes that underwent PPV and in 42 of 52 (80%) eyes that underwent SB (p=0.085). The mean VA improvement in the PPV group was significantly better than in the SB group (PPV, 1.196 logMAR (11 standard ETDRS line); SB, 0.155 logMAR (1.5 standard ETRDS line)) (p<0.001). Subfoveal perfluorocarbon liquid retention was observed in two eyes in the PPV group. Conjunctival pyogenic granuloma occured in one eye in the SB group. Endophthalmitis was not observed in any patient in both groups . Conclusion: Although both surgical methods seems to be efficient treatment modalities for primary regmatogenous retinal detachment, better anatomical outcomes were achieved with PPV. In addition, complication types are different due to different surgical procedures and adjunctive tamponade, scleral buckling elements and perfluorocarbon use. © 2016 Gazi Eye Foundation.
