It is difficult to treat and isotretinoin seems to be the most effective agent. Here we present a 25-year-old man with a facial edema which arose after the complete remission of acne. At least 30% of the diabetic patients have any type of cutaneous manifestation during the course of disease. Some skin conditions are specific to diabetes mellitus however, while most of them may also occur in nondiabetics. In this study, we aimed to investigate the type and frequency of skin lesions in type II diabetic patients, as well as their correlation with HbA1c and body mass index (BMI). One hundred nine type II diabetic outpatients were evaluated for their HbA1c levels, BMI and cutaneous manifestations. At least one skin condition was observed in 74,3 % of the patients. Fungal infections (66 patients, 60,6 %), xerosis (58 patients, 53,2 %), skin tags (43 patients, 39,4 %) and plantar hyperkeratosis (40 patients, 36,7 %) were the mostly observed ones. There was no correlation between skin lesions and age, sex, HbA1c levels and duration of diabetes. Only plantar hyperkeratosis was correlated with BMI (p=0,01). A type II diabetes mellitus is related with obesity and insuline resistance, we observed that plantar hyperkeratosis, skin tags and fungal infections in relation to these situations were frequent in our study group. There was no correlation between HbA1c and skin findings. Though many dermatosis related with diabetes were related, there is no diagnostic skin finding and those seen in diabetics are also observed in nondiabetics.Item Item Lamellar Ichtyosis: Case Report; [Lameller İtiyoz: Olgu Sunumu](2009) Gündüz K.; Kayhan T.Ç.; Gençoǧlan G.; Inanir I.; Temiz P.Introduction: Ichtyosiform dermatoses are a group of hereditary disorders characterized by excessive scaling on the skin. Lamellar ichtyosis is an autosomal recessive disorder. The clinical findings are seen at birth and become widespread and prominent in time; gray-brown scales are seen all over the body. Emollients and keratolytics containing salicylic acid or glycolic acid are used topically. Symptoms are improved significantly by acitretin usage. Case Report: Here we present a 5-year-old girl with lamellar ichtyosis. She was born in a collodion membrane. Besides the skin scales, ectropion and deformities of the nails and ears were observed. Significant clinical improvement was seen with acitretin (10 mg/day) therapy. Conclusion: Lamellar ichtyosis is a severe form of ichtyosiform dermatoses and topical agents may be insufficient. Systemic acitretin treatment improves the symptoms significantly but long term side effects limit its usage. © The Journal of Current Pediatrics, published by Galenos Publishing. All rights reserved.Item Juvenile xanthogranuloma: Case report; [Juvenil ksantogranülom](2010) Gençoǧlan G.; Inanir I.; Demireli P.; Şencan A.; Aygören S.R.Juvenile xanthogranuloma is an uncommon non-Langerhans cell histiocytosis of infancy and childhood. Etiology is not clear. This benign disorder characterized by solitary or multiple yellow-red papulonodules on the skin can occasionally be observed in other organs. They generally appear as reddish-yellow papules which enlarge up to 1 cm in diameter and evolve into yellow-brown plaques and macules. Prognosis is good and lesions mostly improve spontaneously. In this paper, an eleven-month-old boy who admitted with a nodule on the scalp is presented. The possible systemic organ involvement and accompanying diseases were evaluated. No treatment was performed and regular follow-up was planned. Since the lesion did not regress spontaneously during the three months and caused discomfort, total excision was performed. Diagnosis of JXG was confirmed histopathologically. The patches are usually lighter than the surrounding skin, but sometimes may be darker, and are covered by fine, adherent scales. It is very rare in caucasoids, common in Japanese and is also seen in South African blacks and West Indian black and Mediterrean area predominantly in Sardinia have been reported but it is seldom observed in Europe. We present the first two cases diagnosed from Turkey. There were well-demarcated circumscribed scaly plaques with a erythematous periphery and hypopigmented center on the trunk and proximal parts of upper limbs. Their skin phototype was III. Histopathologic examination revealed a moderate degree of compact hyperkeratosis without a granular layer. These are generally cytotoxic, immunosupressive, immunomodulatory and biologic drugs which may have gametotoxic adverse effects. Controlled studies upon male fertility, including the risk of congenital abnormalities are not available; any observed effects are presented as anecdotal evidence. Cyclosporin and methotrexate are the most well-known and frequently used ones. Their effects on spermatogenezis have been reported in several studies. According to the presented data, biologics and retinoids do not have reproductive safety risk. On the contrary, it was found that biologics increase sperm functions by inhibiting TNF-α in vitro studies. The effects of these drugs on male productive system are dependent on therapy period, treatment protocol and the spermatogenetic status of patient before the treatment. Sperm concentration starts to drop about after 2 weeks and reaches a maximum state after 2-3 months. About half of the patients experience a return of sperm production after 12-36 months, independent from dose or therapeutic agent. One year after treatment the drug is completely washed out from the body and there is no increased risk of fathering a malformed child in the first generation. The patients who have to use the drugs that may affect the spermatogenesis negatively should be informed about the risk of decreased fertility, the time to gain the fertility again and cryopreservation. In case of a decrease or loss in fertility, sperm cryopreservation may be an alternative for fertilization. Objective: This study aimed to compare clinical and histopathologic diagnoses of nonmelanoma skin cancers with regard to sensitivity, predictive values, pretest-posttest probabilities, and likelihood ratios. Methods: Two hundred nineteen patients with 241 nonmelanoma skin cancers were enrolled in this study. Of these patients, 49.4% were female and 50.6% were male. The mean age ± standard deviation (SD) was 63.66±16.44 years for the female patients and 64.77±14.88 years for the male patients. The mean duration of the lesions was 20.90±32.95 months. One hundred forty-eight (61.5%) of the lesions were diagnosed as basal cell carcinoma (BCC) and 93 (38.5%) were diagnosed as squamous cell carcinoma (SCC) histopathologically. Results: Sensitivity, positive predictive value, and posttest probability were calculated as 75.96%, 87.77%, and 87.78% for BCC and 70.37%, 37.25%, and 37.20% for SCC, respectively. The correlation between clinical and histopathologic diagnoses was found to be higher in BCC. Conclusion: Knowledge of sensitivity, predictive values, likelihood ratios, and posttest probabilities may have implications for the management of skin cancers. To prevent unnecessary surgeries and achieve high diagnostic accuracies, multidisciplinary approaches are recommended. © 2011 Informa Healthcare USA, Inc.Item Pott's disease with scrofuloderma and psoas abscess misdiagnosed and treated as hidradenitis suppurativa(2011) Ermertcan A.T.; Öztürk F.; Gençoǧlan G.; Nanir I.; Özkütük N.; Temiz P.A 29-year-old man with painless ulcers on the lumbar and inguinal regions associated with purulent discharge of 1.5 years' duration presented to our outpatient clinic. Dermatological examination revealed palpable nodules, discharging sinuses and scars on the left lumbar, gluteal and inguinal regions. According to the clinical, histopathological, scintigraphy, and magnetic resonance imaging findings as well as mycobacteriological examinations, the patient was diagnosed with Pott's disease with scrofuloderma and psoas abscess. On dermoscopic examination of the lesions network structure associated with typical epidermal pigmentation was seen. Histopathologic examination of biopsy specimen taken from the lesions showed superficial hyperkeratosis, papillomatosis and acanthosis in the epidermis, mild perivascular chronic inflammatory cell infiltration in the dermis. The case, resembling acanthosis nigricans, linear verrucous epidermal nevus and zosteriform lichen planus at first sight, was diagnosed as acanthosis nigricans form of epidermal nevus with clinical, dermoscopic and histopathologic findings. The patient was treated with topical retinoic acid and showed significant improvement in the follow ups. Herein, with respect of our interesting case due to the atypical clinical appearence and rarely definition in the literature, clinical features, differential diagnosis, dermoscopic and histopathological findings of acanthosis nigricans form of epidermal nevus will be discussed. Copyright © 2011 by Türkiye Klinikleri.Item Staged-surgery with permanent pathology for the management of high-risk nonmelanoma skin cancer of the nose(2011) Eskiizmir G.; Gençoǧlan G.; Temiz P.; Hirçin Z.; Ermertcan A.The objective of the study was to investigate the effect of staged-surgery with permanent pathology on tumour recurrence of high-risk nonmelanoma skin cancers (NMSCs) of the nose and to determine the factors associated with subclinical spread and deep-margin involvement. Twenty-one patients who underwent staged-surgery with permanent pathology for high-risk NMSCs of the nose between 2007 and 2008 were prospectively followed-up for tumour recurrence. The incidence of tumour recurrence after staged-surgery with permanent pathology was 0%. A positive correlation between perineural involvement and subclinical spread (p = .012); and a statistically significant relationship between infiltrative type basal cell carcinoma (BCC) and deep-margin involvement (p = .033) was detected. Staged-surgery with permanent pathology is a reliable surgical method for complete excision of high-risk NMSCs of the nose and provides a significant decrease in tumour recurrence. Perineural involvement may cause subclinical spread, and infiltrative type BCC may invade deeper structures of the nose. © 2010 Springer-Verlag.Item Ciliary body cysts in neurofibromatosis: A new coexistence?(2012) Emre S.; Palamar M.; Ulusoy M.O.; Gençoǧlan G.Purpose: Neurofibromatosis 1 (NF1) is an autosomal dominant, multisystem disorder that also effects the eye. Herein, we aimed to investigate the posterior iris surface and ciliary body morphology of NF1 patients by ultrasonic biomicroscopy (UB). Materials & Methods: Nine consecutive unrelated subjects with NF1, and as a control group 25 healthy subjects, were included in the study. All patients underwent ophthalmic examination including visual acuity testing, slit-lamp biomicroscopy, tonometry, gonioscopy (Schaffer classification), and dilated ophthalmoscopy, UB. Results: Mean age was 35.1±16.2 (range, 11-57) and 34.5±15.6 (range, 9-60) for NF1 and control groups respectively (p<0.05). Lisch nodules were present in 16 of 18 eyes (88.8%) in NF1 group. Fundoscopic examination of the control group and 15 eyes of NF1 (83.3%) patients was normal, whereas hypoplastic and tilted optic nerve were present in three eyes, and temporally-located bone-spicule-like lesions was present in one eye of the NF1 group. UB revealed ciliary body cyst in 77.7% (14/18) of the eyes among NF1 group, and 8% (4/50) among control group (p<0.05). The mean size of the cysts were 520±191 μ (range, 220-860 μ) and 495± 231 μ (range, 300-830 μ) at NF1 and control groups, respectively. Gonioscopic evaluation revealed that 55% of the NF1 patients have an unoccludable anterior chamber angle (Grade 3 or 4), 45% occludable angle (Grade 1 or 2), and 78% irregular pigment patches. However, occludable angle rate was just 4% in the control group, and none of the patients had irregular pigment patches. Conclusion: The coexistence of ciliary body cysts and NF1, and the effect of these cysts in the eye should be enlightened with further studies. © Springer-Verlag 2011.Item Efficacy of 5% imiquimod cream in a patient with classic kaposi sarcoma(2012) Gündüz K.; Günay Ü.; Inanir I.; Gençoǧlan G.; Temiz P.Background: The classic Kaposi sarcoma is most common in the Mediterranean po-pulation over 50 years of age and presents with reddish-brown papules and nodules particularly on the lower limbs. Treatment depends on the clinical presentation and extension of lesions. Imiquimod is as an immune response modifier with antiangio-genic activity. Main observations: We present a 74-year-old man with classic Kaposi sarcoma who had multiple, small, violaceous papules and nodules on the trunk and extremities with a history of 14 years. He complained particularly from plantar hyperkeratotic painful nodules. Treatment with imiquimod 5% cream under occlusion resulted with almost complete regression within 12 weeks. No local or systemic side effects were observed. Conclusions: Topical imiquimod was a safe and effective therapy in our patient with classic Kaposi sarcoma.Item [The evaluation of tumor histopathology, location, characteristic, size and thickness of nonmelanoma skin cancers of the head and neck].; [Baş ve boyun melanom dişi deri kanserlerinde tümör histopatolojisi, konumu, karakteristiǧi, boyut ve kalinliǧinin incelenmesi.](2012) Eskiizmir G.; Ozgür E.; Temiz P.; Gençoǧlan G.; Ermertcan A.T.This study aims to investigate the characteristic features of tumors and relationship between features in cases who underwent surgery for nonmelanoma skin cancer (NMSC) of the head and neck. Between December 2007 and March 2011, 106 lesions of 90 cases who underwent excision of NMSC of the head and neck in our clinic were included. The statistical analysis was performed by evaluating the demographic data, histopathologic type, size and thickness of tumor, location, Clark stage, T stage, risk of recurrence and the presence of recurrence, reconstruction technique and success rate of surgery. Basal cell carcinoma (BCC) was approximately two-fold more common than squamous cell carcinoma (SCC). The most common locations of tumors were the nose and lip. Most of lesions (71.7%) were at high-risk of recurrence. When the relationship of histopathologic type with the size and depth of tumor were analyzed, it was found that the size and thickness of tumor in SCC were relatively higher with a statistically significant difference. The relationship between histopathologic type and the location of tumors was also statistically significant. Basal cell carcinoma is the most common NMSC of the head and neck. Most of these cases have the potential of high rate of recurrence. The size and thickness of SCC are higher than BCC.
