    Nonmelanoma Skin Cancer of the Head and Neck Clinical Evaluation and Histopathology

    Clinical and histopathologic features of nonmelanoma skin cancer, physical examination, and diagnostic methods (biopsy, dermoscopy, confocal microscopy) are summarized. A diagnostic algorithm provides a useful summarization of differential diagnosis of basal cell carcinoma, actinic keratosis, Bowen's disease, and squamous cell carcinoma.

    Sexual Dysfunction in Female Patients With Neurodermatitis

    Neurodermatitis is a chronic disease affecting the patient's psychosocial status and quality of life. It is associated with a variety of psychologic problems, including demoralization, depression, anxiety, obsessive-compulsive disorder, and sleep disturbances. Coexistence of sexual dysfunction, especially in women, with several systemic diseases has gained interest in recent years. In this study, we evaluated sexual function in female patients with neurodermatitis. We enrolled 89 women (43 patients, 46 controls) in the study. Quality of life was assessed with the Dermatology Life Quality Index (DLQI), and the Female Sexual Function Index (FSFI) was used to determine sexual function. Individuals with psychiatric disorders and/or those using antidepressants were excluded. The total DLQI score was 11.95 +/- 5.65 in patients with neurodermatitis. The total FSFI score was significantly lower in patients compared with healthy controls (22.76 +/- 5.31 and 28.83 +/- 3.50, respectively; P=.001). Domain scores of FSFI (desire, arousal, lubrication, orgasm, and satisfaction) except pain were significantly lower in patients with neurodermatitis (P=.001). The pain score was also lower in patients than controls, but the difference was not statistically significant (P=.073). Neurodermatitis may be associated with sexual dysfunction, and patients with neurodermatitis should be evaluated with regard to sexual function to provide a better quality of life.

    Subungual glomus tumor: surgical and histopathologic evaluation

    Glomus tumor is a rare and benign vascular tumor, arising from neuromyoarterial glomus bodies. It is generally localized on the hand and especially the nail bed and is usually solitary. Pain, tenderness, and temperature sensitivity are the classic triad of symptoms of a glomus tumor. Owing to the small lesions and absence of specific skin features in the nail bed and nail matrix localizations, glomus tumor may not be taken into consideration. In particular, presentation of patients to practitioners of different disciplines for treatment of pain may cause diagnostic delays. Surgical excision is the most effective treatment approach and provides histopathologic diagnosis and rapid improvement of symptoms. This article describes a patient with a subungual glomus tumor for which surgical excision was performed.

    Effects of isotretinoin on spermatogenesis of rats

    Objectives: We evaluated time- and dose-dependent effects of isotretinoin on spermatogenic activity by apoptosis, cyclin D1, E2F, p53 expressions, and Johnsen''s scores. Methods: The rats were divided into three groups. In the 1st group (n == 18), 1 mg/mL/day and in the 2nd group (n == 18) 2 mg/mL/day isotretinoin were administered for 21 days. Flower oil was given to the 3rd (n == 6) control group. On the 7th (groups 1a and 2a), 14th (groups 1b and 2b), and 21st (groups 1c and 2c) days, six rats from the 1st group and six rats from the 2nd group were sacrificed and bilateral orchiectomy was done. Results: The number of cyclin D1 and E2F-positive cells decreased nonsignificantly parallel to days in the 1st group, whereas there was a statistically significant decrease in the 2nd group for the same cells. The p53-positive cells in groups 1c and 2c were increased significantly. Conclusions: Further studies on the effects of retinoids on spermatogenesis should be conducted. It may be wise to administer contraception to male patients, especially during high-dose and long-term retinoid therapy.

    Trichilemmal cyst with homogeneous blue pigmentation on dermoscopy

    A 61-year-old woman was referred to our dermoscopy unit for a pigmented lesion that had been present on her left arm for 8 years. The patient did not notice any enlargement or change in colour. On dermoscopy, homogeneous blue pigmentation was seen. The lesion was excised with the pre-operative diagnosis of melanoma, blue naevus and dermatofibroma. Histopathological examination showed a trichilemmal cyst in the mid-dermis. Although homogeneous blue pigmentation on dermoscopy is the hallmark of blue naevus, it may be seen in metastatic melanoma and exceptionally in hemosiderotic and cellular types of dermatofibroma. Trichilemmal cyst should be borne in mind also in the dermoscopic differential diagnosis.

    Isotretinoin-induced Effects of Mast Cells on Wound Healing

    Wound healing is a complex process, and the role of retinoids in wounds is confusing and controversial. In this study, the authors aimed to evaluate the role of oral isotretinoin on mast cells, collagen production and remodeling in the wound healing process. In the first group 2 mg/kg/day isotretinoin dissolved in sunflower oil were administrated for 28 days. In the second group, only sunflower oil was administered. At the end of first week, four incisions were made on rats' back. In the seventh (group la and 2a), fourteenth (group 1b and 2b) and twenty-first (group 1c and 2c) days, the skin biopsies were taken from around of the incision areas from both groups. The authors revealed that isotretinoin administration permanently accelerates mast cell accumulation parallel to days in the wound area. The histological features of wound healing in isotretinoin administered rats were faster and better than the group 2.

    White fibrous papulosis of the neck

    A 56-year-old white man with multiple, discrete nonfollicular papules on the neck is presented. Clinical and histopathologic features were compatible with the entity of white fibrous papulosis of the neck (WFPN). Pseudoxanthoma elasticum--like papillary dermal elastolysis (PXE-PDE) and WFPN are further clinicopathologic patterns of intrinsic aging. Clinically, WFPN is characterized by isolated, whitish papules, whereas those of PXE-PDE are yellowish and often coalesce to form cobblestone plaques. Our case showed clearly marginated whitish papules. The major histopathologic feature of WFPN is superficial dermal fibrosis with scant elastolysis; in PXE-PDE, there is papillary dermal elastolysis but no sign of fibrosis. No recurrence was performed in the 3 years'' follow-up in our case. Surgical treatment may be considered in such cases with well-circumscribed lesions.

    Imiquimod 5% cream for isolated lichen planus of the lip

    Lichen planus (LP) of the lips is a rare condition that is generally associated with other parts of the oral mucosa. Lip localization has an increased risk, since external trauma, smoking and ultraviolet light trigger malignant transformation. Only a few cases of isolated LP of the lips have been reported up to now, but results of larger series on oral LP suggest that it might be underestimated. Treatment of oral LP is usually difficult and lesions are generally resistant or recur, so that novel therapy alternatives are necessary. Here we report four cases of isolated LP of the lip successfully treated with imiquimod 5% cream. It was applied twice daily, 5 days a week, for 2 weeks. Two weeks after therapy, complete clinical and histopathological resolution was observed. No recurrence was observed during the 5, 10 and 18 months' follow-up period in cases 4, 3 and 1, respectively. Clinical and histopathological cure was also observed in case 2, but the patient showed clinical activation after 6 months. We suggest that imiquimod 5% cream is a safe and effective therapeutic treatment for oral LP.

    Angiokeratoma circumscriptum of the tongue

    Angiokeratoma circumscriptum of the tongue is a rare mucocutaneous vascular disorder. To date, only three cases of angiokeratoma circumscriptum of the tongue have been reported in the English literature. In this article, a case with angiokeratoma circumscriptum of the tongue was presented, and all the aspects of this clinical entity including clinical evaluation, differential diagnosis, histopathological features, and treatment modalities were discussed.

    Hypopyon-like features: new dermoscopic criteria in the differential diagnosis of cutaneous lymphangioma circumscriptum and haemangiomas?

    Background Lymphangiomas are congenital malformations of the lymphatic system characterized by clusters of translucent vesicles. Dermoscopically, yellow and bluish lacunea according to the lymphatic or haematic content were described previously, but it is difficult to distinguish them from haemangioma. Observations In nine patients with cutaneous lymphangioma circumscriptum (CLC), hypopyon-like features were observed. None of the 10 patients with haemangioma had this finding. Conclusion Hypopyon-like features can be a useful dermoscopic pattern in differentiating CLC from haemangioma.

    Evaluation of sequential effect of isotretinoin on the haematological parameters in patients with acne vulgaris

    Purpose: Isotretinoin is the most effective drug for acne with some side effects. Few studies exist regarding the effects of isotretinoin on haematological parameters with different results. Mostly, baseline values with a single value during or at the end of the treatment were compared. In this study, we aimed to determine the differences in haematological parameters during isotretinoin treatment until reaching the cumulative dose of 120mg/kg.Materials and methods: The study included 118 patients with moderate-to-severe acne vulgaris. Patients with preexisting liver disease, anaemia, iron deficiency, abnormal liver function tests, thrombocytopenia/thrombocytosis or hyperlipidaemia were excluded. Laboratory monitoring for haematological parameters was performed at baseline and monthly during treatment. Parameters at the baseline, at the first and second months and at the end of the therapy were taken into account.Results: According to general linear model analysis, platelets and plateletcrit increased at the first month of the treatment and then decreased to baseline. White blood cells and neutrophils decreased at the first month, then increased to baseline value at the second month, and were found to be decreased again at the end of the treatment. Mean corpuscle volume was found to be increased at the end of the treatment. Other parameters in CBC did not show statistically significant differences.Conclusions: Although some changes occur in haematological parameters during isotretinoin therapy, all of these changes remain within the normal range. Evaluating the spot values at any time during treatment may cause misinterpretations.

    Primary cutaneous anaplastic CD30+ large-cell lymphoma that completely regressed after incisional skin biopsy

    We describe a 48-year-old woman with three erythematous nodules localized on the left forearm, with 2 months evolution. Histological and immunohistochemical examination revealed a CD30(+) large-cell lymphoma. Systemic involvement was not detected. The tumor regressed spontaneously within a week, after the incisional skin biopsy. In control skin biopsy, there was not any histological feature of lymphoma. No reactivation or any symptom of systemic disease was observed during the 10-month follow-up period.

    Pyogenic Granuloma in Two Children Successfully Treated with Imiquimod 5% Cream

    Therapeutic Hotline: Treatment of prurigo nodularis and lichen simplex chronicus with gabapentin

    Psychocutaneous conditions are frequently encountered in dermatology practice. Prurigo nodularis and lichen simplex chronicus are two frustrating conditions that are classified in this category. They are often refractory to classical treatment with topical corticosteroids and antihistamines. Severe, generalized exacerbations require systemic therapy. Phototherapy, erythromycine, retinoids, cyclosporine, azathiopurine, naltrexone, and psychopharmacologic agents (pimozide, selective serotonin reuptake inhibitor antidepressants) were tried with some success. Here five cases with lichen simplex chronicus and four cases with prurigo nodularis, who responded well to gabapentin, are presented.

    In Their Own Voices; Living with Hidradenitis Suppurativa: A Qualitative Study

    OBJECTIVE:To provide a deeper assessment of the lived experience of patients with hidradenitis suppurativa, a chronic inflammatory disorder characterized by recurrent abscess, sinuses, and scars.METHODSThis study used a phenomenologic approach. Researchers recruited 10 participants via purposive sampling. Data were collected via semistructured, in-depth, face-to-face interviews, and interpretative phenomenologic analysis was performed.RESULTSData analysis uncovered two primary themes among participants' interview responses: unending symptoms and life becoming hard.CONCLUSIONSThe symptoms of hidradenitis suppurativa negatively affect individuals' family, professional, and daily lives, and patients may benefit from psychiatric intervention.
