Here a 15 year-old girl having both clinic and histopathologic features of pityriasis rubra pilaris, who was classified as type III according to Griffiths (juvenil classical form) and treated with acitretin is presented while the treatment modalities are reviewed.Item Prevalence of skin conditions in primary school children in Turkey: Differences based on socioeconomic factors(2002) Inanir I.; Turhan Şahin M.; Gündüz K.; Dinç G.; Türel A.; Serap Öztürkcan D.Skin diseases are associated with environmental factors and a public health approach is particularly important. We determined the prevalence of skin conditions and associated socioeconomic factors in primary school children in Turkey. A questionnaire for determining the socioeconomic level and a complete dermatologic examination were performed in 785 children in two primary schools in different socioeconomic areas of suburban and central Manisa, Turkey. The study included 345 (43.9%) girls and 440 (56.1%) boys with a mean age of 9.25 ± 1.55 (range 6-14 years). Infectious skin diseases were frequently observed: pediculosis capitis in 74 children (9.4%), scabies in 17 (2.2%), viral skin diseases in 30 (3.8%), and fungal infections in 6 (0.7%). The other common conditions were melanocytic nevi (14.4%), keratosis pilaris (12.5%), pityriasis alba (12%), xerosis (11.8%), and atopic dermatitis (6.8%). Pediculosis capitis, acne, and dandruff were more common in girls. The rate of infections, atopic dermatitis, xerosis, and pityriasis alba were significantly higher in the school children with poor socioeconomic conditions. Improvement in socioeconomic conditions along with education may be needed to decrease the prevalence of some of these skin disorders in order to decrease costs related to treatment.Item Erythema multiforme associated with blister beetle dermatitis(2002) Inanir I.[No abstract available]Item Case report. Tinea pedis and onychomycosis in primary school children in Turkey(2002) Inanir I.; Şahin M.T.; Gündüz K.; Dinç G.; Türel A.; Arisoy A.; Öztürkcan S.A cross-sectional study was performed in two primary schools with different socioeconomic status in the suburban and central areas of Manisa, Turkey, in order to determine the prevalence of tinea pedis and onychomycosis. A full dermatological examination and a questionnaire on socioeconomic conditions were performed in a group of 785 randomly selected children aged 6-14. Of 9 clinically suspected tinea pedis and 4 onychomycoses cases, KOH examination (direct microscopy) and/or mycological cultures were positive in six boys, in whom Candida glabrata and C. tropicalis grew. Older age and higher number of siblings were found to be significant factors for fungal infection.Item Inguinal keratotic basal cell carcinoma mimicking giant solitary trichoepithelioma(Japanese Dermatological Association, 2003) Şahin M.T.; Demir M.A.; Kaya Y.; Can M.; Inanir I.; Öztürkcan S.Keratotic basal cell carcinoma may not only clinically but also histologically share more or less the same features with giant solitary trichoepithelioma. It can be difficult to distinguish these two entities from each other, even for an experienced dermatopathologist. We present an unusual case of inguinal keratotic basal cell carcinoma mimicking giant solitary trichoepithelioma in a 56-year-old woman with a finger-like tumor of 20 years duration. The patient presented with an asymptomatic, skin colored, firm, nonulcerative, nodular lesion. Scanty mitotic activity and apoptotic cells were the histopathologic findings against basal cell carcinoma, whereas absence of papillary mesenchymal bodies, presence of peritumoral lacunae detected only around the solid areas, and accumulation of amyloid-like hyalinized material were the findings in favor of basal cell carcinoma. This case illustrates that keratotic basal cell carcinoma must be taken into account in the differential diagnosis of inguinally located solitary, polypoid masses, especially giant solitary trichoepithelioma.Item Palmoplantar lichen planus presenting with vesicle-like papules(Japanese Dermatological Association, 2003) Gündüz K.; Inanir I.; Türkdogan P.; Sacar H.Palmoplantar lichen planus is a rare, localized variant of the disease that mav create difficulty in diagnosis if it is present as an isolated finding. Although several morphological patterns may be seen, plaques or small papules with compact hyperkeratosis are usually observed. We present a 25-year-old woman with a one-month history of slightly pruritic, red papules on her palms and soles. Dermatologic examination revealed numerous, asymptomatic, unscaly, red papules on her palms and soles, some of them resembling vesicles and white reticulate plaques on both sides of the buccal mucosa. The diagnosis was established by the typical histopathological features of lichen planus. Although resistant to topical corticosteroids, she responded well to systemic corticosteroid therapy, and no recurrence was observed during the follow-up period of one year.Item Solid facial edema of acne: Failure of treatment with isotretinoin(2003) Kilinç I.; Gençoǧlan G.; Inanir I.; Dereli T.Solid facial edema of acne is a rare persistent skin condition which occurs as a late complication of acne vulgaris. It is difficult to treat and isotretinoin seems to be the most effective agent. Here we present a 25-year-old man with a facial edema which arose after the complete remission of acne. Treatment with isotretinoin for 4 months was unsuccessful in this case.Item Developing a quality of life questionnaire in patients with psoriasis; [Psoriasisli hastalarda yaşam kalite ölçeǧi geliştirilmesi](2003) Inanir I.; Aydemir Ö.; Gündüz K.; Danaci A.E.; Ermertcan A.T.Background and design: Psoriasis is a chronic skin disease which causes psychological, social and physical problems and affecting quality of life. Developing a quality of life instrument for patients with psoriasis which is suitable for our country is aimed in this study. Materials and methods: Twenty five patients with psoriasis, 25 relatives and 25 doctors were questionned about daily life problems caused by psoriasis and the mostly defined problems were collected in an questionnaire. The reliability and validity of this questionnaire were analysed in a group of 156 patients. Results: For reliability, Chronbach's α coefficient was 0.87, and item-total correlations were between 0,37-0,60 in internal consistency. There was a high test-retest reliability (r=0.98, p<0.001). For validity, three factors were extracted in the Principal Components Method in construct validity. The maximum factor loadings of each item were between 0.456-0.681. In convergent validity, all patients' self-ratings and PASI were correlated with the questionnaire score (p<0.001). Conclusion: The questionnaire consisting of 17 items were found to be suitable for both epidemiologic and clinical trials.Item Cyclic lymphocytic vasculitis associated with chronic lymphocytic leukemia(2004) Çabuk M.; Inanir I.; Türkdoǧan P.; Ceylan C.; Deǧirmenci M.; Türel A.; Özdemir E.Lymphocytic cutaneous vasculitis associated with a haematological malignancy has rarely been reported. Here, we describe a 61 year-old woman with chronic lymphocytic leukemia (CLL) who presented with cutaneous lesions on both hands. These lesions improved after all combination chemotherapy courses and recurred before each course. Repetitive skin biopsies revealed lymphocytic vasculitis. After 7 courses of chemotherapy, she had a complete remission. Skin lesions disappeared and did not recur. The cyclic pattern of lymphocytic vasculitis and its relation with CLL disease activity are interesting clinical features in this case. © 2004 Taylor & Francis Ltd.Item Bullous pemphigoid associated with prostate adenocarcinoma(2004) Öztürkcan S.; Ermertcan A.T.; Şahin M.T.; Türkdoǧan P.; Inanir I.; Lekili M.Bullous pemphigoid is a common autoimmune skin disease characterized by the presence of subepidermal blisters. It has been associated with underlying neoplasia in isolated reports. A 78-year-old man with generalized blisters was diagnosed as bullous pemphigoid on clinical, histopathological and direct immunofluorescence grounds. His free and total prostate specific antigen (PSA) levels were high and histopathological examination of a prostate specimen revealed prostate adenocarcinoma. We present this rare case to discuss the possible association between bullous pemphigoid and prostate adenocarcinoma.Item Norfloxacin-induced toxic epidermal necrolysis(2005) Sahin M.T.; Ozturkcan S.; Inanir I.; Filiz E.E.OBJECTIVE: To report a case of toxic epidermal necrolysis (TEN) in a man who was treated with oral norfloxacin for prostatitis. CASE SUMMARY: A 40-year-old man presented with a severe skin reaction, which was diagnosed as TEN. He had received norfloxacin 800 mg/day over a 14-day period for prostatitis and, 10 days after finishing the treatment regimen, he developed cutaneous and mucous lesions typical of TEN. After a prolonged hospitalization and treatment with oral prednisolone therapy, fluid resuscitation, and wound dressing, the man recovered. DISCUSSION: TEN is an infrequent, yet often fatal, severe systemic and cutaneous disease that is most often an adverse drug reaction. There are few case reports of TEN induced by fluoroquinolones. A MEDLINE search (1966-February 2005) revealed no reports of toxic epidermal necrolysis, but one incidence of Stevens-Johnson syndrome due to norfloxacin therapy. An objective causality assessment suggests that TEN was probably related to norfloxacin in this patient. CONCLUSIONS: To our knowledge, this is the first case of TEN associated with the use of oral norfloxacin. Although the exact pathogenesis of SD is unknown, Malassezia yeasts as well as genetic and environmental factors have been implicated. The aim of this study was to evaluate the efficacy of terbinafine 1% cream on SD lesions with face localisation. Thirty-five patients with SD were included in the study. Patients applied terbinafine 1% cream twice daily for four weeks. The severity of the signs (erythema, scaling, infiltration) was assessed using a 4-point score (0=absent, 1=mild, 2=moderate, and 3=intense) at baseline and at the 2nd and 4th weeks of the therapy. Also, self-assessment was done by the patients on a 100 mm visual analogue score (VAS) at each visit. Complete remission was observed in 10 (32.3%) patients at the end of the therapy. Statistically significant reductions in the scores of all parameters were observed at both the 2nd and 4th weeks of the therapy. Patients' self assessments in the 2nd and 4th weeks were similarly better than at the baseline.Item Examination of bcl-2 and p53 expressions and apoptotic index by TUNEL method in psoriasis(2006) Gündüz K.; Demireli P.; Vatansever S.; Inanir I.Background: Psoriasis is characterised by hyperproliferation and by aberrant differentiation. Blockage of the normal apoptotic process is one of the factors implicated in the pathogenesis. Objective: To determine the apoptotic features by using TUNEL method and also bcl-2 and p53 expressions in psoriatic epidermis. Materials and methods: Biopsies of 35 patients with psoriasis vulgaris and 14 normal skin were evaluated. Apoptotic cells were detected using the dUTP nick-end labelling assay; bc1-2 and p53 expressions were assessed by using immunohistochemical techniques. A semi-quantitative grading system (HSCORE) was used for comparison. Results: Bcl-2 was strongly expressed in basal keratinocytes of normal skin, while no expression was observed in 13 (37.2%) of the psoriatic samples and it was weakly expressed in the spinous cell layer of 22 (62.8%) samples. Moderate p53 expression was observed in the psoriasis group, while it was weak in the control. The percentage of TUNEL positive cells were significantly increased in the psoriasis group (65±2.30) when compared with the control (32.84±7.16). Conclusion: Apoptotic index besides bc1-2 and p53 expressions in psoriasis differ from normal epidermis. Down-regulation of bc1-2 is consistent with the dynamics of psoriasis but increased TUNEL positive cells and p53 expression has not been fully elucidated yet. © Blackwell Munksgaard 2006.Item Expression of matrix metalloproteinases (MMP-2, MMP-3, and MMP-9) and fibronectin in lichen planus(Blackwell Publishing Ltd, 2006) Gunduz K.; Demireli P.; Inanir I.; Nese N.Background: Keratinocyte damage and lichenoid-interface reaction are the two major pathologic findings in lichen planus (LP). Matrix metalloproteinases (MMPs) are proteinases that participate in extracellular matrix (ECM) degradation and may play an important role in basal membrane (BM) damage in LP. Fibronectin (FN) mediates a variety of cellular interactions with ECM and plays important roles in cell adhesion, migration, growth and differentiation. Objective: To determine MMP-2, MMP-3, MMP-9 and FN expressions in LP and discuss the possible associations. Materials and methods: Skin biopsy samples of 55 patients with LP and 11 normal skin were investigated. Five discoid lupus erythematosus (DLE) and 5 chronic dermatitis (CD) samples were also examined for comparison. Immunochemical stainings were performed for MMP-2, MMP-3, MMP-9 and fibronectin. Results: Weak or absent expressions of MMP-2 and MMP-3 in epidermis; and dense MMP-9 expression in dermal inflammatory infiltrate cells were detected in LP. FN expression was lost in epidermal basal layer and papillary dermis. Conclusion: Loss of MMP-2, MMP-3 and FN in LP can be explained with the destruction of the epidermal basal layer. Conclusion: The questionnaire consisting of 17 items was found to be suitable for both epidemiologic and clinical trials. © 2005 The International Society of Dermatology.Item A case of mycetoma successfully treated with itraconazole and co-trimoxazole(2006) Gündüz K.; Örgüç Ş.; Demireli P.; Inanir I.; Sürücüoglu S.; Ovali G.Y.A 29-year-old woman with swelling, multiple nodules and discharging sinuses of her right foot is presented. A single nodule on the sole was excised 15 years ago and since then she has had recurrent attacks of swelling and discharging sinuses that improved partially with antibiotics. Magnetic resonance images (MRI) revealed an ill-defined mass predominantly with low signal intensity on T2W images. Within the granulomata, multiple unenhancing foci, with low T1W and T2W signal most likely representing the fungal balls or grains were detected. Histopathological examination revealed large clusters of microorganisms resembling fungal hyphae and bacteria, which were surrounded by mixed inflammatory infiltrate cells and stained positively by PAS and Gomori's methenamine silver stain. As minimal regression was seen on MRI with 4 months' itraconazole (200 mg day-1) treatment, co-trimoxazole (160 TMP/800 SMX b.i.d.) was added to treatment. Complete remission was established by MRI examination after 10 months with this combination therapy. © 2006 The Authors.
