Item Primary Duhamel-Martin operations in neonates and infants(2001) Mir E.; Karaca I.; Günsar C.; Sencan A.; Fesçekoǧlu O.Background: With advances in neonatal anesthetic and surgical care, a safe, one stage, definitive procedure has been possible in Hirschsprung's disease. Since 1996, we have performed this type of operation in the neonatal and early infancy period. We aimed to review our data to state the feasibility of this operation in these age groups. Methods: At Dr Behçet Uz Children's Hospital, we treated 10 patients with a single stage Duhamel-Martin operation between 1996 and 2000. Of the 10 patients, seven were boys. Six patients were diagnosed in the first week of the neonatal period. We evaluated these 10 patients by means of age, sex, age at diagnoses, operational age, diagnostic tools, properties of operation, complications and results. Results: The patients were all full-term delivery and had a mean birthweight of 3 kg. The presenting clinical features were abdominal distention (100%), constipation (100%) and vomiting (70%). One patient was a Down syndrome patient, while another patient showed familial Hirschsprung's disease. Contrast enemas gave positive results in eight patients. Definitive diagnoses were performed with rectal biopsy specimens. The extension of the disease was rectosigmoid in nine patients and descending colon in one patient. Five patients were in the newborn period at the time of the operation, while the oldest one was 7 months old. In the postoperative period, two children were treated because of early abdominal eventration and evisceration of the wound. Postoperative enterocolitis occurred in two patients. These 10 patients have been followed-up for a period of 3 years, and spontaneous defecation and weight gain was observed in all of the patients. Conclusions: Our study confirmed the published data that this operation could be performed as an easy and safe procedure in the neonatal and early infancy period.Item Pyloric atresia associated with multiple intestinal atresias and pylorocholedochal fistula(W.B. Saunders, 2002) Sencan A.; Mir E.; Karaca I.; Gunsar C.; Sencan A.; Topcu K.Although congenital pyloric atresia commonly occurs in isolation, it has rarely been reported in association with other alimentary tract atresias. This is a report of a newborn with congenital pyloric atresia associated with duodenal atresia, jejunal atresia, apple-peel ileal atresia, and pylorocholedochal fistula. Preoperative diagnosis was duodenal atresia because of bilious vomiting, and erect radiogram showed double bubble sign. The presented case is the first report of such an association. Saunders, 2002) Gunsar C.; Sencan A.; Karaca I.; Mir E.Isolated esophageal atresia is characterized by a long segment between the 2 esophageal pouches. The authors describe a case of pure esophageal atresia that showed spontaneous recanalization in the waiting period for primary esophageal anastomosis. Regurgitation of gastrostomy formula surprisingly was seen in her mouth. The patency of the esophagus was confirmed with thoracic computed tomography (CT) and an esophagogram. A strictured area in the proximal upper third of the esophagus was managed with baloon dilatation. She was discharged in normal condition, feeding by mouth, and with a normal esophagogram. This interesting event led us to speculate about a different type of pure esophageal atresia with the feasibility of the ends recanalizing after some events such as inflammation. This could be the subtype 115 of type 2 in Kluth's atlas of esophageal atresia in which they described 2 blind esophageal ends and a cyst occupying the intervening space. Another speculation about this subject is the presence of double membranes associated with esophageal atresia with a normal outer appearance of the esophagus as was first described by A. All rights reserved.Item Meckel's diverticulum: Incidence and complications in childhood; [Meckel divertikülü: Siklik ve çocukluk çaǧindaki komplikasyonlar](2003) Günşar C.; Ceylan H.; Munis N.; Karaca I.; Mir E.Aim: Meckel's diverticulum is the most commonly seen congenital anomaly of the gastrointestinal system. Many complications related with this diverticulum could be seen in childhood. The aim of this study is to investigate the incidence of Meckel's diverticulum in our population and to evaluate the features of the complications originating from it. Method: The data about 1685 patients who underwent laparotomy in İzmir Behcet Uz Children's Hospital between 1990-1996 were investigated. The patients with Meckel's diverticula were classified as symptomatic or asymptomatic. The parameters evaluated were age, sex, clinical presentation, associated anomalies, type of complications related with Meckel's diverticulum, ectopic mucosa, surgical treatment and complications. Results: 25 patients were classified as asymptomatic whereas 35 were classified as symptomatic in a group of 60 patients with Meckel's diverticulitis. The average age was 4.7 years. The incidence was calculated as 1.51 %. The male female ratio (m/f) was 2.5/l and 2.6/l for symptomatic and for asymptomatic patients respectively. Intestinal obstruction (18 cases) was the most common clinical finding presented with vomiting and abdominal pain which were the primary symptoms. Fifteen diverticulum in the asymptomatic group, and all of them in the symptomatic group were excised. The ratio of ectopic mucosa was found as 16 % for the asymptomatic, and 51 % for the symptomatic cases. Symptomatic group had a 16 % complication rate. Conclusion: Meckel's diverticulum is a congenital structure, which is frequently encountered with the clinical presentation of intestinal obstruction in childhood causing life-threatening complications it should be investigated in the laparotomies performed for acute abdomen and where appendices were found innocent.Item Blunt renal injuries in Turkish children: A review of 205 cases(2003) Ceylan H.; Gunsar C.; Etensel B.; Sencan A.; Karaca I.; Mir E.A retrospective analysis of the medical records of 205 children with renal injuries secondary to blunt abdominal trauma is used to make recommendations regarding the initial diagnostic and therapeutic approaches in this type of patient. It was found that the absence of hematuria on initial urinalysis does not exclude a serious renal injury. Thus, following blunt abdominal trauma, all children should undergo imaging procedures to exclude renal injury, whether they have hematuria or not. Ultrasound is a good initial screening procedure in all patients. Computed tomography is recommended for the definitive evaluation of suspected major renal injuries. Since even major renal injuries may heal without surgical intervention, conservative management is the recommended initial treatment of choice. Surgery is reserved for those children who are hemodynamically unstable and those that develop complications.Item Paraesophageal hiatal hernias in children(2003) Yazici M.; Karaca I.; Temir G.; Etensel B.; Günşar C.; Güçlü C.; Mutaf O.Hiatal hernia is a rare condition, which may be a cause of important clinical problems either as a mediastinal mass or as a cause of failure of the antireflux mechanism. Nineteen patients treated for paraesophageal hiatal hernias were included in the study. We investigated demographic data, diagnostic studies and symptoms of the patients together with the type of operation and outcome. Respiratory and gastrointestinal complaints were the prominent symptoms in most patients. Plain X-ray, contrast radiological study and esophagoscopy were used in the diagnostic workup. Surgical repair was performed via thoracic, abdominal or thoraco-abdominal approaches. Concomitant antireflux procedures were performed in 13 patients. Hiatal hernias in children may be asymptomatic or may present with a variety of symptoms or dramatic complications. Because of the risk of complications, surgical treatment is necessary shortly after diagnosis. Repair of the hiatus combined with antireflux surgery seems to yield satisfactory results.Item Atresia of the colon(W.B. Saunders, 2005) Etensel B.; Temir G.; Karkiner A.; Melek M.; Edirne Y.; Karaca I.; Mir E.Background/Purpose: Colonic atresia (CA) is one of the rarest causes of neonatal intestinal obstructions, and no large series can be reported. Therefore, we did perform a retrospective clinical trial to delineate our CA cases and carry out a literature survey. Methods: We reviewed the charts of CA cases treated in our center between 1992 and 2002. We aimed to collect all reported cases in Medline, and personal communications with the authors of published series were used to reach the missing data. Results: The chart review revealed 9 newborns with CA treated in our center (6 cases of type III, 2 cases of type II, and 1 case of type IV). These accounted for 3.7% of all gastrointestinal atresias managed in our center. Of the CA cases, 3 were isolated and 6 had at least one or more associated congenital anomalies. The preferred surgical technique at the initial treatment of CA was performing a proximal stoma and distal mucous fistula in an average of postnatal 59.4 hours. The literature survey enabled us to reach 224 cases of CA, including our cases. Conclusions: Because of the low incidence of CA, delay in diagnosis and treatment may occur. The mortality is statistically higher when the surgical management is performed after 72 hours of age. However, the prognosis of CA is satisfactory if diagnosis and surgical management could be made promptly and properly. © 2005 Elsevier Inc. All rights reserved.Item Treatment and prophylaxis with sucralfate ameliorates hypoxia/ reoxygenation-induced intestinal injury in pup rats(2005) Sencan A.B.; Sencan A.; Aktas S.; Habif S.; Kabaroglu C.; Parildar Z.; Karaca I.Background: Sucralfate is widely used as a cytoprotective agent in patients with peptic ulcer and other intestinal mucosal injury. The aim of this study is to investigate whether sucralfate has any effect on the prevention and treatment of hypoxia/reoxygenation-induced intestinal injury. Material/Methods: Four groups of 10 1-day-old rat pups were studied. Hypoxia/reoxygenation (H/O)-induced intestinal injury was created. Group 1 was subjected to H/O just after birth and sacrificed at the end of the third day (Treatment Control). Group 2 was subjected to H/O just after birth and treated with sucralfate for 3 days. They were sacrificed at the end of the third day (Treatment). Group 3 was subjected to H/O on the third day after birth and then sacrificed (Prophylaxis Control). Group 4 was treated with sucralfate for the first 3 days, then H/O was created. Just after H/O, the pups were sacrificed (Prophylaxis). The intestinal tissues were harvested for histopathological investigation. Malondialdehyde (MDA) levels in the intestinal tissues were determined. Results: The mucosal injury grades of the treatment and prophylaxis groups were significantly lower than those of control groups (p<0.05). The mean MDA level in the treatment and prophylaxis groups were 0.42±0.17 and 0.21±0.23 nmol/mg respectively. The MDA levels of both groups were significantly lower than in the control groups (p<0.05). Conclusions: The present study shows that sucralfate has beneficial effects in an experimental model of hypoxia/reoxygenation-induced intestinal injury. © Med Sci Monit, 2005.Item The evaluation of influencing factors of "quality of life" in the elderly in Muradiye District, Manisa; [Manisa Muradiye bölgesi̇ndeki̇yaşlilarin yaşam kali̇tesi̇ni̇etki̇leyen faktörleri̇n deǧerlenḋiṙilmeṡi](2007) Cengiz Özyurt B.; Eser E.; Çoban G.; Akdemir S.N.; Karaca I.; Karakoç Ö.Introduction: The aim of this study was to determine the components of quality of life and the variables which influence these components in the people 65 and ovef, living in Muradiye district, Manisa. Materials and Method: A representative sample of 200 older adults among the total population of 613 inhabitants over 65 were studied in Muradiye district, Manisa: The quality of life was evaluated by using World Health Organization Quality of Life Questionnaire (WHOQOL-OLD) that has six domain structure. Student's t test and logistic regression analysis were performed in data analysis. Results. The mean age of the respondents were 71.21±5.01, 63.5% Were Married, 43.7% were illiterate, 70.0% reported any chronic illness, 84.0% were living under support of family and 16.6% have no health insurance. The mean values of the domain scores of the WHOQOL-OLD were as follows: Sensory-Abilities 11.15±2.32; Autonomy 12.72±2.51; Past-Present-and-Future-Activities 12.39±2.33; Social-Pardcipation 11.52±2.59; Death-and-Dying 10.84±3.33 and Ifitimacy 14.37±2.00. The mean values for the total module score were 3.04±0.32. Conclusion: Multivariate analysis showed that the most effective variables on quality of life of elderly were education level and need of support during daily activities.Item Zooplankton composition of Van Lake Coastline in Turkey(Academic Journals, 2010) Yildiz S.; Özgökçe M.S.; Özgökçe F.; Karaca I.; Polat E.This study was completed with periodical surveys in coastal band of Van Lake between May and September in 2005 to 2006 period. It was carried out on a total of 20 sites selected as sampling points and having three distinct characteristics. During the study, a total of 20 species (14 belonged to genus of Rotifera, 4 to Copepoda and 2 Branchiopoda) were found at the Van Lake coastal zone. Because of ecologial factors that cause high level fluctuation in population density, It is possible to get zero individual at samplings, and on the other hand, differences between maximum and minimum values obtained in different samplings intervals can be very high. Because this type of data based on counting does not show normal distribution, and shape of the distribution is skewed to the right because of the abundance of zero, using the Zero-inflated Poisson regression method (ZIGP) is required. This study was carried out to obtain information on effects of physico-chemical environmental conditions on population fluctuation of Notonecta viridis. Samplings were conducted with monthly periods along the coastal band of Van Lake in 2005-2006. Samples were taken from 20 sampling places where have three different characters as stream entrances, settlements and natural coastlines. Reults were analysed by using ZIGP regression model. According to results, Effect of sampling intervals and sampling stations on population densities of Notonecta viridis were important. On the other hand, HCO3 had negative effect on population densities in zero-inflated model while it had possitive effect on population densities in other two models. It was determined that Fe effected the species populations in the negative way in the mean regression model, and Cl and Mg effected it in possitive way in the overdispersion regression. In the result, it was deductived that Notonecta viridis was found excessive numbers or none in some sampling stations because of the pysico-chemical structures of water.Item Cohomology theory for digital images(2013) Ege O.; Karaca I.In this paper we propose a mathematical framework that can be used for dening cohomology of digital images. We state the Eilenberg-Steenrod axioms and the Universal Coefficient Theorem for this cohomology theory. We show that the Künneth formula doesn't hold. Moreover, we prove the Lefschetz fixed point theorem for a digital image. We then give some examples about the fixed point property. Rosenfeld [23] introduced the notion of a digitally continuous function between digital images, and showed that although digital images need not have fixed point properties analogous to those of the Euclidean spaces modeled by the images, there often are approximate fixed point properties of such images. In the current paper, we obtain additional results concerning fixed points and approximate fixed points of digitally continuous functions. Among these are several results concerning the relationship between universal functions and the approximate fixed point property (AFPP). © AGT, UPV, 2016.
