Item The Role of Endoplasmic Reticulum Stress in Gastroesophageal Reflux Disease Symptoms and Treatment(2023) Erdal BALCAN; Elmas Kasap; TAHİR BURAN; AYSUN TORAMAN; Pinar Solmaz Hasdemir; Mehmet Korkmaz; Çağdaş AKTANBackground: Gastroesophageal reflux disease is a common condition worldwide. There is no curative treatment for gastroesophageal reflux disease. Endoplasmic reticulum stress leads to the activation of the unfolded protein response and has an important role in inflammation. The aim is to determine the role of endoplasmic reticulum stress in the follow-up of individuals with gastroesophageal reflux disease and the temporal changes of endoplasmic reticulum stress markers with treatment. Methods: Twenty-four subjects in total were recruited prospectively, of whom 15 had nonerosive reflux disease. Two biopsies from 2 cm above the esophagogastric junction, 2 biopsies from gastric antrum mucosa, and 2 biopsies from gastric corpus mucosa were taken. Simultaneously, 2 tubes of venous blood samples were drawn from each individual (1 tube for studying the genetic markers and 1 tube for analyzing the CYP2C19 polymorphism). Results: The mean age was 42.3 ± 17.6 for women and 34.66 ± 11.2 for men. Pantoprazole, esomeprazole, rabeprazole, and lansoprazole preparations were used for treatment. There was no significant difference between tissue and blood samples for panel genes ATF-6, XBP-1, DDIT-3, DNAJC-10, and EIF-2-AK before treatment. There was a significant decrease in the level of ATF-6, XBP-1, DNAJC-9, EIF- 2-AK, and NF-2L-2 genes in blood after treatment. In the comparison of proton pump inhibitors, significant decreases in the expression of the ATF-6, XBP-1, and DNAJC-9 mRNAs were detected in blood from individuals after treatment. Conclusion: Endoplasmic reticulum stress can be for evaluating the clinical improvement and the effectiveness of treatment in gastro- esophageal reflux disease.Item Image-Guided Percutaneous Drainage Reduces the Need for Surgical Interventions in Patients with Tubo-Ovarian Abscess: A Cohort Study(2023) gökhan pekindil; Fatih Düzgün; Duygu Uçar Kartal; beyhan cengiz özyurt; Pinar Solmaz HasdemirObjective: Management of tubo-ovarian abscess (TOA) is a challenging healthcare problem especially in reproductive age women. The aim of this study is to determine the current role of image-guided primary percutaneous drainage in avoidance of surgical intervention in the management of TOA. Material and Methods: A total of 76 patients hospitalized in our tertiary care center with the diagnosis of TOA were retrospectively evaluated. The study population was divided into 2 groups based on the treatment modalities as antibiotic treatment (n=48) and image-guided percutaneous drainage (n=28) and evaluated in terms of clinical and laboratory characteristics and the requirement for surgical intervention. Results: Surgical intervention was required in 1 (3.6%) patient treated with percutaneous drainage and in 10 (20.8%) patients treated with antibiotics (p=0.036). The choice of treatment modality was independent of demographic characteristics, clinical and laboratory findings. The size of TOA in percutaneous drainage group was significantly larger compared to the antibiotic treatment group (mean 6.75±1.886 cm versus 5.92±5.88 cm, respectively, p=0.047). Rehospitalization during follow-up was higher among patients treated with antibiotics only compared to percutaneous drainage group (p=0.06). Conclusion: Percutaneous drainage is an acceptable treatment option including large-sized TOA and decreases the requirement of surgical intervention in selected cases.Item Genç Kadında Adneksiyal Kitle ile Karışabilen İntestinal Ewing Sarkomu(2024) Pinar Solmaz Hasdemir; Aygul Aliyeva; Hanife Seda Mavili; Gamze GOKSELİskelet dışı Ewing sarkomu, oldukça nadir görülür ve intestinal Ewing sarkomu literatürde sırasıyla 20 kez ve omental Ewing sarkomu 2 kez bildirilmiştir. Bu olgu sunumu, yaygın karın ağrısı şikayeti ile başvuran ve ovaryan kitle ön tanısı ile opere edilen 23 yaşında kadın hastaya aittir. Hastaya yapılan abdominal ultrasonografi ve intravenöz kontrastlı batın tomografide adneksiyal alandan başlayarak bağırsak ansları arasına doğru uzanım gösteren kitle saptandı. Tümör markerları yükselmişti [karbonhidrat antijen-125 (CA-125): 427,5; karsinoembriyojenik antijen (CA-19.9): 67,9]. Laparotomide ince bağırsak mezosundan köken alan ve tümü ile omentum ile kaplanmış kitle total olarak eksize edildi ve histolojik olarak Ewing sarkomu tanısı kondu. Bu olgu intraabdominal kitle ile başvuran genç kadın hastada nadir görülen iskelet dışı Ewing sarkomunun ayırıcı tanılarda yer alması gerektiğinin önemini vurgulamaktadır.
