Item Leiomyoma as a cause of urinary retention(Page Press Publications, 2013) Sarsmaz K.; Goker A.; Kuscu N.K.Urinary retention is uncommon in reproductive age women. The frequent causes for obstructive retention are pelvic masses and a history of surgery. We report a case of intermittent urinary retention caused by a leiomyoma. A 47-year old patient with voiding difficulties for the past four months was referred to the gyne-cology outpatient clinic. She had urinary retention and had been catheterized several times for bladder emptying. She had been evaluated by the urology clinic and a cystoscopy was performed with normal findings. Magnetic resonance imaging (MRI) revealed a pelvic mass at the anterior uterine wall with a diameter of 10 cm. Her physical examination confirmed the MRI findings. Total abdominal hysterectomy was performed with a complete resolution of the patient's complaints. Women with urinary retention should be consultated with a gynecologist in order to rule out pelvic masses. © Copyright K. Sarsmaz et al., 2013.Item Rectus abdominis muscle endometriosis(College of Physicians and Surgeons Pakistan, 2014) Goker A.; Sarsmaz K.; Pekindil G.; Kandiloglu A.R.; Kuscu N.K.Endometriosis is characterized by an abnormal existence of functional endometrial tissue outside the uterine cavity, typically occuring within the pelvis of women in reproductive age. We report two cases with endometriosis of the abdominal wall; the first one in the rectus abdominis muscle and the second one in the surgical scar of previous caesarean incision along with the rectus abdominis muscle. Pre-operative evaluation included magnetic resonance imaging. The masses were dissected free from the surrounding tissue and excised with clear margins. Diagnosis of the excised lesions were verified by histopathology.Item Immunohistochemical and ultrastructural analysis of the effect of omega-3 on embryonic implantation in an experimental mouse model(Elsevier Ltd, 2016) Sarsmaz K.; Goker A.; Micili S.C.; Ergur B.U.; Kuscu N.K.Objective: Implantation is the first step to a healthy pregnancy. Omega-3 supplementation is common to use during pregnancy, for its antioxidant and membrane stabilising effect. In this study we have aimed to study the effect of Omega-3 supplementation on implantation in a mouse model by immunohistochemical methods and electron microscopic evaluation. Materials and methods: Mice were randomized into three groups to receive standard food, Omega-3 400 mg/kg and Omega-3 1000 mg/kg one menstrual cycle before mating. Mice were sacrificed on third day of estimated implantation and uterine horns were evaluated immunohistochemically for staining of Laminin and Leukemia Inhibitory Factor (LIF) and ultrastructural morphology. Results: Laminin and LIF immunoreactivity were increased signifcantly in the high dose group when compared to the control and low-dose groups in lumen epithelium basal membrane, gland epithelium basal membrane and endometrial stroma. Electron-microscopic evaluation showed a decrease in epithelial height and microvilli loss in the high dose groups. Conclusion: Omega-3 supplementation increased implantation markers Laminin and LIF and decreased epithelial height and microvilli thus seems to prepare the endometrium for a favorable environment of implantation. © 2016.Item The Relationship between Serum Neuropeptide FFR2, Serum Smoothelin and Pregnancy Outcomes in Pregnant Women with Gestational Hypertension; [Gestasyonel Hipertansiyonu Olan Gebelerde Serum Nöropeptid FFR2, Serum Smoothelin ve Gebelik Sonuçları Arasındaki İlişki](Turkish Society of Cardiology, 2024) Taş S.; Sarsmaz K.; Sarsmaz H.Y.; Gürgen S.G.; Taş Ü.; Eyüboğlu M.; Arı Z.Objective: Gestational hypetension is a major public health concern due to its links with cardiovascular disease, stroke and neonatal morbidity and mortality. Timely diagnosis and effective management of gestational hypertension are essential for both maternal and neonatal health. Neuropeptide FF Receptor 2 (NPFFR2) is a protein secreted by the brain and placenta, involved in pain regulation, water balance, and the modulation of cardiovascular effects. This study aims to conduct a comparative analysis of NPFFR2, smoothelin (SMTH), echocardiographic results and pregnancy outcomes in pregnant women with and without gestational hypertension. Method: This study included 78 pregnant participants, which were grouped into women with gestational hypertension (n = 39) and those without gestational hypetension (n = 39). The gestational hypertension population was classified into two groups, i.e., dipper and non-dipper groups, based on the 24-hour ambulatory blood pressure monitoring results. Smoothelin and NPFFR2 analyses were performed using the blood samples and placental tissue samples collected from all participants, along with echocardiography and 24-hour ambulatory blood pressure monitoring. Results: The study group comprised 78 pregnant women with a mean age of 28.8 years and mean gestational age of 27.7 weeks. The gestational hypertension group had a significantly higher NPFFR2 levels, lower SMTH levels and gestational age at birth and higher all 24-hour ambulatory blood pressure monitoring findings. The left atrial-to-aortic ratio and Tricuspid annular plane systolic excursion (TAPSE) were significantly higher in GİH group than in the control group. NPFFR2 and gestational age at birth were found to be independently associated with neonatal intensive care unit admission. Conclusion: Serum NPFFR2 levels were increased in women with gestational hypertension, SMTH levels were decreased, and pregnancy prognosis was found to be associated with NPFFR2 levels. © 2024 Turkish Society of Cardiology. All rights reserved.Item Delta neutrophil index (DNI) as a potential biomarker for fetal growth restriction: insights from maternal hematological changes and neonatal outcomes(BioMed Central Ltd, 2024) Tonyali N.V.; Sarsmaz K.; Bayraktar B.; Kahraman N.C.; Sucu S.T.; Aktemur G.; Cakir B.T.; Seyhanli Z.; Karabay G.; Cakir A.; Ustun Y.Background: This study investigates the role of Delta Neutrophil Index (DNI), an inflammation marker, in late-onset fetal growth restriction (LO-FGR) and its prediction of composite adverse neonatal outcomes. Methods: A retrospective study was conducted on 684 pregnant women (456 with normal fetal development and 228 with LO-FGR) who delivered at Health Sciences University Etlik Zubeyde Hanim Women’s Health Training and Research Hospital between January 1, 2015, and June 30, 2018. Composite adverse neonatal outcomes were defined as at least one of the following: 5th minute APGAR score < 7, respiratory distress syndrome (RDS), or neonatal intensive care unit (NICU) admission. Results: The FGR group had significantly higher levels of neutrophil to lymphocyte ratio (NLR), platelet to lymphocyte ratio (PLR), monocyte to lymphocyte ratio (MLR), and DNI compared to controls (p < 0.05, for all). For FGR diagnosis, the DNI demonstrated the highest area under the curve (AUC = 0.677, 95% CI: 0.642–0.711) with a cut-off value of > -2.9, yielding a sensitivity of 78.41%, a specificity of 52.97%, a positive likelihood ratio (+ LR) of 1.68, and a negative likelihood ratio (-LR) of 0.37 (p < 0.001). For predicting composite adverse neonatal outcomes in the FGR group, DNI again demonstrated superior performance with an AUC of 0.635 (95% CI: 0.598–0.670), a cut-off value of > -2.2, a sensitivity of 69.90%, a specificity of 55.36%, a + LR of 1.56, and a -LR of 0.51 (p < 0.001). NLR, PLR, and MLR had AUCs below 0.55, indicating poor discriminative ability, with none reaching statistical significance. Conclusion: This study highlights the potential role of DNI as a promising biomarker for detecting inflammatory processes associated with LO-FGR and its complications. © The Author(s) 2024.Item Atraumatic forceps-guided insertion of the cervical pessary: a new technique to prevent preterm birth in women with asymptomatic cervical shortening(Walter de Gruyter GmbH, 2024) Dereli M.L.; Obut M.; Özkan S.; Sucu S.; Fıratlıgil F.B.; Kurt D.; Kurt A.; Sarsmaz K.; Tolunay H.E.; Çağlar A.T.; Üstün Y.E.Objectives: As previous studies on the use of a cervical pessary to prevent preterm birth (PTB) have produced conflicting results, we aimed to investigate the feasibility, acceptability and safety of a new technique for inserting a cervical pessary and compare it with the traditional technique in patients at high risk of PTB. Methods: Women at high risk of PTB treated with a cervical pessary between January 2018 and January 2021 were retrospectively evaluated. After applying exclusion criteria, a total of 68 eligible patients were identified and retrospectively analyzed. The primary outcome was spontaneous PTB before 34 weeks’ gestation (WG). Results: Of 68 participants, 39 were treated with the traditional method (group 1) and 29 with the new insertion technique (group 2). The rate of spontaneous PTB before 34 WG was significantly lower in group 2 (p=0.020). Birthweight, APGAR scores and satisfaction with the method were significantly higher, while PTB before 37 WG was significantly lower in group 2 (p=0.043, 0.010, 0.009, 0.042 and 0.014, respectively). There were no significant differences in the rates of perinatal death (12.8 vs. 3.4 % in groups 1 and 2, respectively; p=0.229). The concomitant use of vaginal progesterone was required more frequently in group 1. According to the binary regression analysis, the new insertion technique resulted in a 5.42 and 3.97-fold protection against PTB before 34 and 37 WG. Conclusions: Our preliminary results show that our new technique of pessary insertion is more effective than the traditional method in preventing PTB due to cervical shortening. © 2024 Walter de Gruyter GmbH. All rights reserved.Item Does Second Trimester Maternal Serum Zonulin Level Predict Gestational Diabetes Mellitus?(Multidisciplinary Digital Publishing Institute (MDPI), 2024) Tonyalı N.V.; Arslan B.; Sucu S.T.; Sarsmaz K.; İbanoğlu M.C.; Örgül G.; Aktemur G.; Yücel A.; Şahin D.Zonulin, a protein that regulates intestinal permeability, has attracted attention as a potential biomarker for GDM. Therefore, this study aims to investigate whether there are differences in zonulin levels between the GDM group and control groups, especially between those receiving different treatments (diet and insulin). Based on this idea, we included 90 patients with a gestational age between 24 and 28 weeks in our study. While GDM was not detected in 33 of these patients, as a result of OGTT, 57 patients were diagnosed with GDM and these patients were followed throughout their pregnancy. Gestational diabetes was diagnosed by an OGTT performed between 24 and 28 weeks of gestation according to American Diabetes Association (ADA) standards. During follow-up, GDM patients were divided into two groups according to whether they required insulin treatment. Plasma zonulin levels were determined using enzyme-linked immunosorbent assay (ELISA) techniques. The GDM group had significantly higher plasma zonulin levels than the control group (p < 0.005). According to our research, zonulin may be a non-invasive biomarker involved in the etiology of GDM. Large-scale research on this topic is still needed. © 2024 by the authors.
