    The hemodynamic responses to hypothermic and normothermic cardiopulmonary bypass

    This prospective randomized clinical study was designed to assess and compare the effects of normothermic and hypothermic nonpulsative cardiopulmonary bypass in patients with mitral valve replacement. Forty patients undergoing elective mitral valve replacement were randomly divided into two groups according to the temperature of perfusion. Group N (15 patients) underwent normothermic cardiopulmonary perfusion and Group H (25 patients) underwent hypothermic cardiopulmonary perfusion. These groups were compared using clinical and electrocardiographic criteria and cardiovascular evaluation parameters. Quantitative data were analyzed using the paired Student t test. While there was a significant increment in heart rate in Group N in relation to preoperative values, there was no change in heart rate in Group H. Increasing values in systemic vascular resistance (SVR) parameters were detected in both groups. Oxygen consumption (VO2) values decreased in Group N in relation to Group H. There was no significant change in alveolar-arterial O2 difference (DO2) values between these two groups. These results indicate that the temperature of cardiopulmonary perfusion had no effect on the immediate postoperative values but did cause an increase in heart rate in the normothermic group.

    Antibody Sustainability in SARS-CoV-2 Healthcare Professionals’ Patient Cohort; [Sağlık Çalışanları SARS-CoV-2 Hasta Kohordunda Antikor Sürdürülebilirliği]

    In this study, it was aimed to evaluate one-year follow-up of severe acute respiratory syndrome coronavirus-2 (SARS-CoV-2) specific antibodies formed against the virus binding site, in a coronavirus disease-2019 (COVID-19) positive case cohort (n= 413) between the period March 2020 to December 2020 in Manisa Celal Bayar University Hospital, until July 2021. SARS-CoV-2 antibodies were determined by the chemiluminescent enzyme immunoassay (CLIA) method. Values of 1.0 and above were considered positive. Chi-square tests and Joinpoint regression analysis (version 4.7.0) were used in the statistical analyses. The mean age of the participants was 34.9 ± 9.3 and 60.2% of them were women. Between 21-30 days after the diagnosis of COVID-19, total antibody level was above the threshold value in 72.2% (n= 126) of the participants, while this rate increased to 79.1% (n= 240) in 31-60 day interval. In the following period, this rate decreased to 38.8% (n= 108) in days 211st to 240th. Antibody response could not be detected in 76 (20.7%) of 367 employees who have initially been followed up. The percentage of total antibody positivity prevalence ranged from 98.9% to 96.1% in the 31-210th day after diagnosis, in the follow-up of 291 employees whose total antibody positivity was detected after diagnosis. According to the results of the Joinpoint regression analysis, after the diagnosis of COVID-19, the curve showing the percentage of antibody positivity was broken at two points: The first breaking point was observed in 181-210th days (6-7 months) (p= 0.069), and the second breaking point was in 271-300th days (9-10 months) (p< 0.001). As a result, the highest antibody positivity rates were detected after the 30th day of the disease onset and antibody positivity was maintained in the first seven months after diagnosis; the antibody positivity rate decreased to 25% at the end of the first year. © 2022 Ankara Microbiology Society.     Distinctive sleep complaints and polysomnographic findings in antibody subgroups of autoimmune limbic encephalitis

    Introduction: Sleep disturbances are being increasingly recognized in association with autoimmune encephalitis (AIE). We investigated the prevalence of sleep-related symptoms and polysomnographic features of patients with AIE and the long-term outcomes in these patients in a multi-center, prospective study from Turkey. Methods: We prospectively evaluated patients with definite AIE in a common database including demographics, AIE-related and sleep-related symptomatology. Maximum and latest modified Rankin scores (mRS) and Liverpool Outcome Score (LOS) were noted. Results: Of 142 patients, 87 patients (61.3%) fulfilled the criteria for definite AIE (mean age, 46.8+18.8 years; 51.7% women; mean disease duration, 21.0+38.4 months). 78.9% of patients had at least one or more new onset or worsened sleep-related symptomatology: insomnia (55.3%), excessive daytime sleepiness (EDS, 28.0%), sleep apnea (18.7%), REM sleep behavior disorder (RBD, 17.3%), restless legs syndrome (10.7%) and oneiric stupor (9.3%). Sleep efficiency, N3 and REM sleep were decreased and N1 sleep was increased in patients with Ab[+] AIE. LOS points were highest in those with insomnia and sleep apnea, and lowest in those with EDS, RBD and oneiric stupor. RBD and sleep apnea were more common in anti-LG1 Ab[+] group than anti-NMDAR Ab[+] group. Index of periodic leg movements was highest in anti-LG1 Ab[+] group. Patients with EDS and oneiric stupor had more common memory problems. Maximum and latest mRS scores were positively correlated with EDS and oneiric stupor. EDS, RBD and oneiric stupor were negatively correlated with LOS points. Conclusion: Our study emphasizes the presence and importance of early diagnosis of sleep disturbances in AIE in regard to their deteriorative influences on disease prognosis. © The Author(s) 2024.
