The purpose of the symposium was to address the issue of whether or how the current system of naming pleomorphic fungi should be maintained or changed now that molecular data are routinely available. The issue is urgent as mycologists currently follow different practices, and no consensus was achieved by a Special Committee appointed in 2005 by the International Botanical Congress to advise on the problem. The Declaration recognizes the need for an orderly transitition to a single-name nomenclatural system for all fungi, and to provide mechanisms to protect names that otherwise then become endangered. That is, meaning that priority should be given to the first described name, except where that is a younger name in general use when the first author to select a name of a pleomorphic monophyletic genus is to be followed, and suggests controversial cases are referred to a body, such as the ICTF, which will report to the Committee for Fungi. If appropriate, the ICTF could be mandated to promote the implementation of the Declaration. In addition, but not forming part of the Declaration, are reports of discussions held during the symposium on the governance of the nomenclature of fungi, and the naming of fungi known only from an environmental nucleic acid sequence in particular. Possible amendments to the Draft BioCode (2011) to allow for the needs of mycologists are suggested for further consideration, and a possible example of how a fungus only known from the environment might be described is presented. © 2011 International Mycological Association.Item The analysis of violence against the nurses who are in employee status in Mugla State hospital, Turkey(2012) Picakciefe M.; Akca S.; Elibol A.; Deveci A.; Yilmaz N.; Yilmaz U.E.Objektive: Workplace violence in the co untry, especially in the health system remains obscure. The search of the relationship using an intersectional, analytical survey between the working conditions and violence exposure of the nurses who work in Muǧla State Hospital was aimed in the study. Materials and methods: Of the 310, 268 nurses participated, yielding a response rate of 86.5%. The data was examined by using Fisher's Exact Test, Pearson Chi-Square Test and Logistic Regression. In result, it was found that 85.8% of the nurses were exposed to violence, with 70.4% of the violence coming from the relatives of patients. Results: 77.2% of the nurses were exposed to verbal abuse while 71.4% were physically assaulted. Unfortunately, 98.4% of the abused did not report the physical abuse. 91.3% of the nurses always felt violence anxiety, while 92.9% of them thought that their instutions do not make an effort for the security systems. According to logistic regression analysis, having worked 21 years and over, night work and having felt a violent encounter in the workplace, significantly increases the frequency of meeting with violence of nurses. Conclusion:In conclusion we found that the nurses were exposed to multiple forms of violence. This was correlated with their monthly average income, marrital status, parental status, work status, night shifts, overtime, rotational shifts, total and daily work hours.Item Leadership and the function of discourse on international political economy(IGI Global, 2013) Gurel P.A.; Yilmaz R.; Cambaz N.O.; Yilmaz N.This study focuses on the concept of leadership. It aims to explain the relationship between language and leadership. In this theoretical essay, firstly, the concept of leadership is discussed and then the relationship between semiotics, language, and discourse is described. Taking the obtained theoretical frame as a base, the function of language as a sign of leadership is studied within the frame of leadership approaches. The relationship stated in the study provides a theoretical route map for the academicians conducting empirical studies in this field, and this is the reason why this study is authentic and unique. With the effects of globalization rapidly increasing, the considerations of leadership have also changed, and new leadership typologies have emerged to supply the needs of the information age. Both at home and abroad, language skills and good command of language provide a global and competitive advantage for leaders, and this increases the significance of acquiring the skills mentioned. © 2014, IGI Global.Item Frequency of respiratory viruses in children with lower respiratory tract infection; [Alt solunum yolu enfeksiyonu olan çocuklarda solunum yolu viral etkenlerinin sıklıǧı](Kare Publishing, 2013) Akçali S.; Yilmaz N.; Güler Ö.; Şanlidaǧ T.; Anil M.Aim: Lower respiratory tract infections (LRTI) have high morbidity rates in children. In this study, it was aimed to investigate the prevalence of respiratory viruses in children with LRTI symptoms. Material and Method: A total of 160 children who were diagnosed with LRTI between October 2009 and March 2010 were included into the study. The presence of respiratory syncytial virus (RSV) (A+B), influenza virus (A+B), parainfluenza virus (PIV) (1, 2, 3, 4), human metapneumovirus, rhinovirus and coronavirus (OC43+229E) in throat swab samples were investigated by real-time PCR The RealAccurateTM Respiratory RT PCR Kit (PathoFinder B.V., Netherlands). Results: In 67 samples (41.8%), at least one virus which could cause acute respiratory tract infection was found. Overall, RSV was the most frequently identified virus (52.2%), followed by rhinovirus (26.8%), coronavirus (5%), metapneumovirus (2.9%) and PIV 1 (1.4%). As the other viral agents, coronavirus was detected in 4 samples (5%), hMPV was detected in 2 samples (2.9%) and PIV was detected in 1 sample (1.4%). When the frequency of coinfections was evaluated, RSV- rhinovirus association was found in 4 samples, RSV-coronavirus association was found in 1 sample, rhinovirus-coronavirus association was found in 1 sample and RSV-rhinovirus- Coronavirus association was found in 1 sample. Conclusions: In 41.8% of the study group, a viral factor responsible for the clinical signs was detected. Indoor fungal contamination in hospital air is associated with a wide range of adverse health effects. Regular determination of fungal spore counts in controlled hospital environments may help reduce the risk of fungal infections. Because infants have inchoate immune systems, they are given immunocompromised patient status. The aim of the present study was to evaluate culturable airborne fungi in the air of hospital newborn units in the Thrace, Marmara, Aegean, and Central Anatolia regions of Turkey. A total of 108 air samples were collected seasonally from newborn units in July 2012, October 2012, January 2013, and April 2013 by using an air sampler and dichloran 18% glycerol agar (DG18) as isolation media. We obtained 2593 fungal colonies comprising 370 fungal isolates representing 109 species of 28 genera, which were identified through multi-loci gene sequencing. Penicillium, Aspergillus, Cladosporium, Talaromyces, and Alternaria were the most abundant genera identified (35.14, 25.40, 17.57, 2.70, and 6.22% of the total, respectively). © 2017, Springer International Publishing AG.Item Comparison of Different Kinesio Taping Techniques After Third Molar Surgery(W.B. Saunders, 2020) Gözlüklü Ö.; Ulu M.; Gözlüklü H.Ö.; Yilmaz N.Purpose: Elastic therapeutic taping method has been shown to reduce pain and edema after surgery. The purpose of the present study was to compare the effects of 2 different Kinesio taping (KT) techniques on swelling, pain, and trismus after third molar surgery. Patients and Methods: The study was designed as a split-mouth, single-blinded, and controlled randomized clinical trial. Patients undergoing lower impacted third molar extraction at the Department of Oral and Maxillofacial Surgery, Faculty of Dentistry, İzmir Katip Çelebi University, were included in the present study. Group I included 30 patients and was used to compare the classic KT technique (technique A) and the new KT technique (technique B). Group II included 15 patients and was used to compare the classic KT technique (technique A) and no KT (No-KT [control]). Group III included 15 patients and was used to compare the new KT technique (technique B) and No-KT (control). Swelling was assessed using the 3dMD Face System (3dMD, Atlanta, GA). The maximum interincisal distance was recorded using a digital caliper preoperatively (T0) and at postoperative days 2 (T1) and 7 (T2). The visual analog scale for pain scores were recorded postoperatively at 30 minutes, 1, 2, 3, 4, 5, 6, 8, 10, 12, and 24 hours, and 1, 2, 3, 4, 5, 6, and 7 days. The effects of operative time, patient age, and patient gender were also evaluated. Results: The data from 60 patients (27 men and 33 women) with a mean age of 22.28 years (range, 18 to 31 years) were analyzed in the present study. The amount of swelling and the interincisal distance were less with technique B than with technique A and no KT (control group). Conclusions: KT is a useful method for reducing postoperative morbidity after impacted third molar extraction. We conducted this study to assess the demographics, etiologic distribution, treatment patterns and complication frequency of patients with chronic hypoparathyroidism in Turkey. This is a retrospective, cross-sectional database study, with collaboration of 30 endocrinology centers located in 20 cities across seven geographical regions of Turkey. A total of 830 adults (mean age 49.6 ± 13.5 years; female 81.2%) with hypoparathyroidism (mean duration 9.7 ± 9.0 years) were included in the final analysis. Hypoparathyroidism was predominantly surgery-induced (n = 686, 82.6%). The insulting surgeries was carried out mostly due to benign causes in postsurgical group (SG) (n = 504, 73.5%) while patients in nonsurgical group (NSG) was most frequently classified as idiopathic (n = 103, 71.5%). The treatment was highly dependent on calcium salts (n = 771, 92.9%), calcitriol (n = 786, 94.7%) and to a lower extent cholecalciferol use (n = 635, 76.5%) while the rate of parathyroid hormone (n = 2, 0.2%) use was low. Serum calcium levels were most frequently kept in the normal range (sCa 8.5–10.5 mg/dL, n = 383, 46.1%) which might be higher than desired for this patient group. NSG had a lower mean plasma PTH concentration (6.42 ± 5.53 vs. 9.09 ± 7.08 ng/l, p < 0.0001), higher daily intake of elementary calcium (2038 ± 1214 vs. 1846 ± 1355 mg/day, p = 0.0193) and calcitriol (0.78 ± 0.39 vs. 0.69 ± 0.38 mcg/day, p = 0.0057), a higher rate of chronic renal disease (9.7% vs. 3.6%, p = 0.0017), epilepsy (6.3% vs. 1.6%, p = 0.0009), intracranial calcifications (11.8% vs. 7.3%, p < 0.0001) and cataracts (22.2% vs. 13.7%, p = 0.0096) compared to SG. Methods: Eleven experienced centers from our country participated in this retrospective study. The clinical and hormonal features, tumor imaging, pathological results, treatment modalities, and disease courses of the patients were evaluated. Results: 28 female and 26 male patients with ECS were compared. The mean age at diagnosis, clinical characteristics, and hormonal evaluation results were similar. However, insulin-requiring diabetes mellitus (p = 0.04) and osteoporosis with fractures were more common in males (p = 0.03). While more patients with increased DHEA-S levels than the upper limit of normal were found to be higher in females, central hypothyroidism were higher in males (p = 0.02). At the diagnosis, 36 NENs (68% of females and 69% of males) were localized. Small cell lung carcinoma was higher in males (p = 0.02), and the frequency of other NENs was not different. Curative surgery was performed on 61% of females and 46% of males. Tumor size, Ki-67 labeling index, positive ACTH immunostaining, local lymph node and distant metastasis rates were similar in both sexes. In the follow-up, the tumor became visible in 7 of 10 females and 4 of 8 males after medical treatment and/or bilateral adrenalectomy. The remission rates (65% of females, 62% of males) and NEN-related death rates (14% of females, 30% of males) were similar. In individuals with ankylosing spondylitis (AS), CT-P13 has been shown to be effective and to have a well-tolerated safety profile. The aim of this study was to evaluate the long-term drug persistence, safety, and efficacy of infliximab biosimilar CT-P13 in patients with AS undergoing first-line (1st-line) and later (≥2nd-line) treatment in clinical practice. Methods: We performed an observational cohort study that included AS patients based on the biological drug database in the TURKBIO Registry between 2014 and 2021. The patients were divided into two groups: those receiving CT-P13 as first-line treatment or as a switch (≥2nd-line) from another TNF inhibitor (TNFi). Standard disease activity metrics were used to assess the effectiveness of CT-P13, and drug retention rates were investigated. Results: There were 179 AS patients using CT-P13 (47.4% male, mean age: 42.9 ± 11.3 years). Of these patients, 123 (68.7%) were receiving CT-P13 as a first-line treatment. The mean length of treatment was 3.5 years. CT-P13 drug retention rates in the general patient population were 58.6% and 48.2% in the first-line and ≥second-line treatment, respectively, after 3 years of follow-up. The most common reason for CT-P13 treatment discontinuation was lack of efficacy. The first-line CT-P13 group had statistically substantially higher ASAS20/40 response rates at three and six months. Nonetheless, both groups’ response rates at one year were comparable. Conclusions: In this real-world data analysis, AS patients who were TNFi naïve (1st-line) and subsequently treated (≥2nd-line) with CT-P13 showed encouraging drug retention rates with acceptable long-term effectiveness and safety. © 2024 by the authors.Item Relationship of psoriatic arthritis with nail and scalp involvement in Turkish psoriasis patients: Multicentered cross-sectional study(Lippincott Williams and Wilkins, 2024) Askin O.; Engin B.; Ozdede A.; Kartal S.P.; Ugurlu S.; Akbulut T.O.; Ekinci A.P.; Aydogdu İ.H.; Ozden M.G.; Kok H.; Dogan S.; Ozturkcan S.; Borlu M.; Baskan E.B.; Yilmaz N.; Ak T.; Topkarci Z.; Serdaroglu S.Psoriasis is a common multisystem inflammatory disease, and arthritis is an essential component of the disorder, requiring early diagnosis and prompt treatment for successful management. In this study, we aimed to investigate the relationship between nail and scalp involvement and other covariates with psoriatic arthritis (PsA). This cross-sectional study, conducted from June 2021 through December 2021, included 763 patients from 11 different centers in Turkey. The severity of involvement was evaluated using psoriasis area severity index (PASI), nail psoriasis severity index (NAPSI), and psoriasis scalp severity index (PSSI) scores. Predictors for PsA were evaluated using univariate and multivariate logistic regression models. PsA (n = 155, 21.5%) was significantly more common in patients having a family history of psoriasis (43.2% vs 30.9%, P = .004), nail involvement (68.4% vs 52.3%, P < .001), and coexistence of nail and scalp involvement (53.7% vs 39.6%, P = .002). Furthermore, patients with PsA had considerably higher PASI (7 vs 5.6, P = .006), NAPSI (5 vs 2, P < .001), and PSSI scores (7 vs 4, P = .002) and longer disease duration (months) (126 vs 108, P = .009). In multivariate analysis, female gender [OR: 3.01, 95% CI (1.861-4.880), P < .001], nail involvement [OR: 2.06, 95% CI (1.293-3.302), P = .002)], and body mass index (BMI) [OR: 1.06, 95% CI (1.017-1.100), P = .005] were identified as independent predictors for PsA. Female gender, nail involvement, and high BMI are significant predictors for PsA and warrant detailed rheumatological assessment. Notably, being female is the strongest predictor of increased risk of PsA in our survey. Scalp involvement appears not to be associated with PsA. Also, the presence of PsA seems related to a more severe skin involvement phenotype. Copyright © 2024 the Author(s). The aim of the present study was to evaluate asthma in the elderly population, to compare disease characteristics between patients diagnosed <60 (aged asthma) and ≥60 (elderly asthma) years of age. Materials and methods: The study was a prospective, multicenter, cross-sectional type. A questionnaire was filled out to patients 60 years of age and over, that have been followed for asthma for at least 3 months. Asthma Control Test (ACT), eight-item Morisky Medication Adherence Scale (MMAS-8) was filled out, inhaler device technique was assessed. Results: A total of 399 patients were included from 17 tertiary care centers across the country. Mean age was 67.11 years and 331 (83%) were female. The age at asthma diagnosis was ≥60 in 146 (36.6%) patients. Patients diagnosed ≥60 years were older (p < 0.001), had higher education level (p < 0.001), more commonly had first-degree relative with asthma (p = 0.038), asthma related comorbidities (p = 0.009) and accompanying rhinitis/rhinosinusitis (p = 0.005), had better asthma control (p = 0.001), were using less controller medications (p = 0.014). Inhaler technique was correct in 37% of the patients with no difference in between the groups. Treatment compliance was better in elderly asthma patients (p < 0.001). In the multivariate logistic regression analysis, having well-controlled asthma (odds ratio = 1.61, CI = 1.04–2.51), and high medication adherence rate (odds ratio = 2.43, CI = 1.48–4.0) were associated with being in the elderly asthma group. Conclusion: The characteristics of asthma are different among patients aged 60 years and over which seems to be related to onset age of asthma. In our cohort, the elderly asthma patients had higher education level, and treatment adherence and asthma control was better. Patients diagnosed ≥60 years of age did not have more severe disease. © 2024, Turkiye Klinikleri. All rights reserved.
