Item Acute phase reactants in patients with coronary slow flow phenomenon; [Koroner yavaş akım fenomeni olan hastalarda akut faz reaktanları](AVES, 2010) Madak N.; Nazli Y.; Mergen H.; Aysel S.; Kandaz M.; Yanik E.; Çekdemir D.; Tavli T.Objective: In this study, we sought to investigate the serum levels of high sensitivity C-reactive protein (Hs-CRP), N-terminal pro-brain natriuretic peptide (NT proBNP), erythrocyte sedimentation rate, leukocyte, thyroid hormone and fibrinogen levels in patients with coronary slow flow phenomenon (CSFP). Methods: A total of 82 patients with angiographically proven normal coronary arteries and slow coronary flow in all three coronary vessels (45 males and 37 females, mean age 59±11 years) and 34 patients with normal coronary arteries and normal coronary flow (19 males and 15 females, mean age 56±10 years) with similar risk profiles were included in this cross-sectional observational study. Coronary flow rates of all patients and control subjects were documented by Thrombolysis In Myocardial Infarction (TIMI) frame count, serum level of Hs-CRP, NT proBNP, sedimentation, leukocyte, free triiodothyronine (FT3), free thyroxine (FT4), thyroid stimulating hormone (TSH) and fibrinogen levels were measured. Statistical analysis was performed using t test for independent samples, Chi-square test and Pearson correlation analysis. Results: Hs-CRP (0.88±0.86 vs 0.36±0.35 mg/L, p<0.001) and NT proBNP (117.83±163.2 vs 47.33±30.6 ng/ml, p<0.01) were found to be significantly higher in patients with coronary slow flow compared with normal control group. There are no significant differences regarding thyroid hormones, fibrinogen, sedimentation and leukocyte between two groups. The mean TIMI frame counts were positively correlated (r=0.454, p=0.001 and r=0.554, p=0.001, respectively) with plasma Hs-CRP levels and NT-proBNP levels. Conclusion: Hs-CRP and NT proBNP are significantly higher in patients with coronary slow flow compared with normal control group. Their increased levels are positively correlated with TIMI frame count. © 2010 by AVES Yayi{dotless}li{dotless}k Ltd.Item Comparison of audiovisual sexual stimulation test with nocturnal penile tumescence test in the evaluation of erectile dysfunction by using RigiScan; [RigiScan ile erektil disfonksiyonun deǧerlendirilmesinde nokturnal penil tümesans testi ile vizüel seksüel stimülasyon testinin karşılaştırılması](AVES, 2010) Incirci M.; Üçer O.; Karataş T.C.; Ateşçi Y.Z.; Gümüş B.Objective: Nocturnal penile tumescence (NPT) test is a time-consuming, cumbersome, and relatively expensive method which is used for the diagnosis of erectile dysfunction (ED). The aims of the study were to clarify the relationship between erotic and nocturnal erections and to evaluate the diagnostic efficacy of the audiovisual sexual stimulation (AVSS) test with and without medication. Materials and methods: A total of 30 patients with ED were examined. Each patient filled in an International Index of Erectile Function-5 (IIEF-5) questionnaire. AVSS and NPT tests were performed using the RigiScan Plus. AVSS test was performed with and without medication (sildenafil citrate 50 mg). Patients were divided into two groups (psychogenic and organic groups) according to NPT and AVSS test results. Results: The mean age of the patients was 46.4 (23-69) years and IIEF-5 score was 10.8±4.5. Positive AVSS test was found in 18 (85.7%) of the 21 patients with positive NPT test. Negative AVSS test was found in 7 (77, 8%) of the 9 patients with negative NPT test. The specificity of AVSS test was 85.7% and sensitivity was 77.8% for the psychogenic groups. The overall accuracy rate was 83.3%. Conclusion: The NPT test is more expensive, cumbersome, and discomfortable than the AVSS test. Although efficiency of both tests is similar, the AVSS test seems more advantageous. We suggest that the AVSS test without medication should be performed as the first step in the diagnosis of the patients with ED. In patients with negative test result, the AVSS test with medication should be performed as the second step diagnostic method.Item Item Relationship of systemic antiinflammatory markers and functional parameters in chronic obstructive pulmonary disease; [Kronik obstrüktif akciǧer hastalıǧında fonksiyonel paramatrelerle sistemik antiinflamatuar belirteçlerin ilişkisi](AVES, 2011) Satar S.; Alpaydin A.Ö.; Var A.; Coşkun A.Ş.; Çelik P.; Yorgancioǧlu A.Objective: Airway inflammation and oxidative stress biomarkers increase in chronic obstructive pulmonary disease (COPD) and this is thought to be related to faster decrease in lung functions. Systemic markers of oxidative stress include nitric oxide (NO) and malonyl dialdehyde (MDA), while antioxidant markers are erythrocyte superoxide dismutase (SOD) and gluthathionperoxidase (GSH-Px). We investigated the relationship between levels of systemic oxidative/antioxidant markers and functional parameters. Material and Method: Fifty-one COPD patients without any systemic inflammation were included in the study. Pulmonary function test parameters were evaluated. 6-minute walk test (6MWT) and dyspnea severity were recorded. Twelve healthy persons in a control group were also included in the study. Venous blood specimens of the patients and the control group were collected and serum NO, MDA and erythrocyte SOD, GSHPx levels were measured. Results: Mean age of the patients were 62.94±10.56 years. When the control group and COPD patients were compared; MDA (p<0.001), NO (p=0.012) and GSH-Px (p=0.012) were significantly higher in the control group than COPD patients. However, when the age factor was controlled, only MDA(p=0.017) was found higher in the control group. As forced expiratory volume in 1st second (FEV1) decreased; NO, SOD, MDA and GSH-Px increased, although there was no statistical significance. Like FEV1, 6MWT and dyspnea severity by VAS did not show any statistical significance with the parameters investigated. Conclusion: From the results, we could not observe the reflections of oxidant/antioxidant imbalance on functional parameters in COPD. This may be due to incomplete systemic response to local disorders as well as the attempts of antioxidant systems to maintain an equilibrium resembling that in healthy individuals.Item Antimicrobial resistance of gram-negative bacteria isolated from lower respiratory tract specimens of hospitalized patients; [Hastanede yatan hastaların alt solunum yolu örneklerinden izole edilen gram-negatif bakterilerde antimikrobiyal direnç](AVES, 2011) Gazi H.; Ecemiş T.; Kurutepe S.; Gürsev N.; Sürücüoǧlu S.Objective: In this study, we aimed to determine the distribution of the genera of Gram-negative bacteria isolated from lower respiratory tracts of patients treated as in-patients and to calculate the antibiotic resistance rates to guide empirical antibiotic therapy. Methods: Samples taken from the lower respiratory tract and sent to the Bacteriology Laboratory at the Celal Bayar University Faculty of Medicine from January 2008 to December 2010 were retrospectively analyzed. Results: A total of 853 Gram-negative bacteria were isolated from specimens of in-patients. The rates of carbapenem (85%) and multidrug resistance (47%) among A. baumannii were higher than those observed for P. aeruginosa (30% and 19.2%). Carbapenem resistance was not found among the bacteria of the family Enterobacteriaceae, while the highest rates of resistance were detected for ciprofloxacin and trimethoprim-sulfamethoxazole. Conclusions: Implementation of effective treatment protocols based on sensitivity test results can be useful in preventing nosocomial lower respiratory infections caused by resistant Gram-negative bacteria.Item The treatment of late-onset hypogonadism; [Geç başlayan hipogonadizmde tedavi](AVES, 2014) Üçer O.; Gümüş B.Late-onset hypogonadism (LOH) in aging men is a clinical and biochemical syndrome caused by an age-related decline in testosterone. Despite published in guidelines and recommendations, uncertainty surrounds the profile of clinical symptoms as well as the biochemical threshold of diagnosis. The only evidence-based treatment of late-onset hypogonadism is testosterone replacement therapy. The actual available evidence of the long-term risks and outcomes of testosterone-replacement therapy remains very limited, and carefully designed placebo-controlled trials of testosterone administration to assess the risks and benefits of such a therapy are required. Until such evidence is available, testosterone treatment should be restricted to elderly men with very low testosterone levels in the presence of clinical symptoms, and the advantages and disadvantages need to be accurately assessed. Careful monitoring of potential side effects is necessary. The purpose of this review is to discuss what is known and what remains unclear with respect to the benefits and risks of testosterone replacement treatment. © 2014 by Turkish Association of Urology.Item A novel association between TGFβ1 and ADAMTS4 in coronary artery disease: A new potential mechanism in the progression of atherosclerosis and diabetes(AVES, 2015) Uluçay S.; Çam F.S.; Batır M.B.; Sütçü R.; Bayturan Ö.; Demircan K.Objective: Coronary artery disease is characterized by atherosclerosis in the vessel wall. Recently, it has been thought that increasing LDL-binding capacity of subendothelial proteoglycan fragments that are formed by protease activity can be responsible for the initiation of atherosclerosis. ADAMTS4 is a member of the versican-degrading proteinases. In vitro studies demonstrated that TGFb inhibits the expression of ADAMTS4 in macrophages. In this study, we aimed to investigate the role and association between TGFβ1 and ADAMTS4 in coronary artery disease. Methods: A total of 84 cases with atheroma plaque and 72 controls without plaque were analyzed. The severity of disease was determined by Gensini score. TGFβ1 gene polymorphisms were genotyped by the PCR-RFLP method. TGFβ1 and ADAMTS4 serum levels were measured by ELISA method. Statistical analyses of genotypes and their relationship with serum levels were performed by chi-square, student t test and ANOVA. Results: ADAMTS4 levels were higher in cases compared with controls (p<0.05). In the patient group, ADAMTS4 levels were higher than in controls and correlated with TGFβ1 serum levels (r=0.29; p<0.05) and severity of disease (r=0.20; p<0.05). The TGFβ1 gene CCA haplotype was associated with 3.3-fold increase in coronary artery disease (OR=3.26 95% CI 1.22-8.68; p<0.05). Unexpectedly, ADAMTS4 serum levels were also higher in diabetic cases (p=0.05). Conclusion: This study has demonstrated that ADAMTS4 may be responsible for the pathogenesis of atherosclerosis. This is the first report about the association between ADAMTS4 and TGFβ1 serum levels in the progression of atherosclerosis in CAD. Furthermore, it is seen that TGFβ1 haplotype can cause a genetic susceptibility to CAD in the Turkish population. To our knowledge, this is also the first report suggesting higher serum ADAMTS4 levels in diabetic patients. © 2015 by Turkish Society of Cardiology.Item Normal ranges of biorbital and interorbital distances in healthy Turkish pregnancies at 19–23 weeks of gestation and correlation with craniofacial structures(AVES, 2015) Pala H.G.; Artunç-Ülkümen B.; Koyuncu F.M.; Uyar Y.; Bülbül-Baytur Y.Objective: We aimed to determine the normal ranges for biorbital (BOD) and interorbital distances (IOD) during the second trimester in Turkish women with normal pregnancies and to assess the correlation between BOD, IOD, and other fetal craniofacial structures and biometric parameters. Material and Methods: Our retrospective study comprised 1328 women with singleton normal pregnancies who had undergone ultrasonography (USG) examinations at 19–23 weeks of gestation in the second trimester screening. The measurements of BOD and IOD were obtained with the coronal section of the fetal face at the plane of orbits. Results: Mean BOD was 3.4±0.33 cm, whereas mean IOD was 1.28±0.24 cm. Correlation analysis revealed that BOD was significantly correlated with IOD, transcerebellar diameter (TCD), cisterna manga (CM), nuchal fold (NF), nasal bone (NB), biparietal diameter (BPD), head circumference (HC), abdominal circumference (AC), femur length (FL), and gestational week. There was a significant relation between IOD and the lateral ventricle posterior horn, TCD, CM, NF, NB, BPD, HC, AC, and FL. Conclusion: The reference ranges obtained in our study enabled accurate evaluation of BOD and IOD in the second trimester of normal pregnancies. USG detection of fetal orbital biometric anomalies may alert the clinician for different anomalies associated with abnormal development of eye. © 2015 by the Turkish-German Gynecological Education and Research Foundation.Item Life quality of children with congenital heart diseases(AVES, 2018) Sertçelik T.; Alkan F.; Yalın Sapmaz Ş.; Coşkun Ş.; Eser E.Aim: We aimed to evaluate the effects of the severity and symptoms of congenital heart disease and the emotional and behavioral problems of affected children on their quality of life. Material and Methods: The study was performed by interviewing 80 children aged between 6 and 16 years (40 with cyanotic heart disease and 40 with acyanotic disease) and their mothers. A sociodemographic data form, quality of life questionnaire, strength and difficulties questionnaire, and family life and parenting attitudes scale were used in the research. Life quality was the dependent variable of this research. Results: No significant relation was found between age, sex, and education level of the parents and all quality of life subscale scores (p>0.05). In terms of quality of life, total quality of life subscale, emotional well-being and self-esteem subscales were significantly lower in children with cyanotic congenital heart disease (p=0.02, p=0.007, p=0.006, respectively). The total quality of life subscale was significantly lower in children with a medical treatment and surgical history. In terms of clinical symptoms, self-esteem, friendship and school life quality subscales were affected in the presence of dyspnea. As scores from strength and difficulties questionnaire increased, which is used for the assessment of children’s psychological symptoms, all quality of life scores were significantly lowered except for school and family subscales. Conclusion: It was found that symptoms of congenital heart disease affected the psychosocial quality of life subscales rather than the physical subscales. In addition, it was observed that mental symptoms in both the mother and child negatively affected quality of life rather than disease-related parameters. © 2018 by Turkish Pediatric Association.Item Does a smartphone application make it easier to evaluate the dizziness handicap inventory?(AVES, 2019) Uz U.; Uz D.; Çelik O.Objective: The Dizziness Handicap Inventory (DHI) is a questionnaire that is frequently used for patients with vestibular system disorders. The aim of this study was to evaluate the feasibility and reliability of the “e-DHI” smartphone application, which calculates DHI scores. Methods: Web-based iOS and Android smartphone applications (e-DHI) were developed to evaluate the DHI. After completing the questionnaire, the DHI total score and subscores (physical, functional and emotional) were assessed and results were noted and sent to the e-mail address. Twenty-five otorhinolaryngologists and neurologists were enrolled in the study to apply the DHI to the authors sequentially using the conventional assessment method and the e-DHI (randomized by priority). The questionnaire was applied by both methods, and for each method, the time required to complete the questionnaire, to calculate the DHI score, and the total time required were noted. Results: Three of the 25 doctors were excluded from the study due to a calculation error with the conventional evaluation method. The mean total time required for completing and scoring the questionnaire was shorter when the DHI was evaluated with the e-DHI application (160±32.2 seconds) compared with the classic assessment method (289±65.9 seconds) (p<0.001). There was no significant difference between the two methods in terms of the time taken to complete the questionnaire (p=0.269). However, the calculation periods were statistically significantly shorter with the digital application compared with the conventional method (p<0.001). Conclusion: The present study showed that the DHI could be applied more quickly, more comfortably, and safely (with higher accuracy) using the e-DHI smartphone application. © 2019, AVES. Some stool tests can be easily used in primary care in the differential diagnosis of disorders such as gastrointestinal infections, malabsorption syndromes, and inflammatory bowel diseases. Stool tests can prevent unnecessary laboratory investigations. Stool analyses include microscopic examination, chemical, immunologic, and microbiologic tests. Stool samples can be examined for leukocytes, occult blood, fat, sugars (reducing substances), pH, pancreatic enzymes, alpha-1 antitrypsin, calprotectin, and infectious causes (bacteria, viruses, and parasites). Stool should also be macroscopically checked in terms of color, consistency, quantity, shape, odor, and mucus. © 2019 by Turkish Pediatric Association.Item A Comparison of Smokers, Non-Smokers, and Those Undergoing Treatment in Terms of Attitudes on Coping with Stress and Psychological Resilience(AVES, 2019) Acar S.; Kaylı D.Ş.; Yararbaş G.This study compares the attitudes on resilience and coping with stress of smokers, non-smokers, and those undergoing smoking-cessation treatment. This study involves three study groups: 116 people who applied to the Institute on Drug Abuse, Toxicology, and Pharmaceutical Science’s Smoking Cessation Polyclinic in Ege University to stop smoking, 110 smokers, and 121 non-smokers. The individual information form has been prepared in order to measure the variables of gender, age, and education level. In addition to the individual information form, the Fagerström Test for Nicotine Dependence, the Brief Coping Styles Inventory (Brief COPE), and the Resilience Scale for Adults have been used. The Kruskal-Wallis and Mann-Whitney U tests have been utilized in addition to descriptive statistics in analyzing the obtained data. Based on multiple comparisons among the three groups, significant differences among the groups have been obtained in certain sub-dimensions for the variables in coping strategies and resilience, as well as significant differences between smokers and the other groups in terms of their levels of resilience. © 2019 Turkish Green Crescent Society.Item Efficacy of epley maneuver on quality of life of elderly patients with subjective BPPV(AVES, 2019) Uz U.; Uz D.; Akdal G.; Çelik O.OBJECTIVES: This study aimed to evaluate the efficacy of the repositioning maneuver on quality of life in elderly patients with dizziness and/or vertigo. MATERIALS and METHODS: This controlled, prospective randomized clinical trial was conducted in elderly patients aged 65 years and above with a positive history of benign paroxysmal positional vertigo (BPPV), presence of vertigo, and no observable nystagmus during the Dix-Hallpike test, so-called Subjective BPPV (S-BPPV). Individuals were evaluated by visual analog scale (VAS) and dizziness handicap inventory (DHI). Groups were defined as treatment (treated with Epley maneuver bilaterally) or no treatment control (no treatment modality or canalith repositioning maneu-ver). Ten days after the first assessment, all patients were reassessed using VAS and DHI. RESULTS: A total of 50 patients were randomized into two groups: 25 to the treatment group, and 25 to the control group. No significant differences were observed for baseline VAS and total DHI scores between the groups (p=0.636, p=0.846, respectively). On the other hand, after the reassessment, VAS and total DHI scores were both significantly reduced in the treatment group (p<0.001, p<0.001, respectively), but no reduction in either score was found in the control group (p=0.216, p=0.731, respectively). CONCLUSION: This study showed that elderly patients with S-BPPV benefit from the Epley maneuver, in particular global and disease-specific quality of life. © 2019, AVES. All rights reserved.Item Use of tobacco products in turkish children and young people: Is there an alarm for hookah use?(AVES, 2020) Göktalay T.; Coşkun A.Ş.; Havlucu Y.; Horasan G.D.OBJECTIVES: In the recent years, it has been observed that the use of hookah, a tobacco product, has increased in children and young people in Turkey. This study was conducted to determine the prevalence of cigarette and hookah use in children and young people specific to age and gender and to define the factors relevant to the use of hookah. MATERIALS AND METHODS: The study was conducted with secondary/high school students between 11 and 17 years of age (Survey 1, n=4718) and with university students between 18 and 23 years of age (Survey 2, n=1588) as two groups, which made a total of 6306 students. The students were asked to fill in a questionnaire which consisted of questions about socio-demographic data and the use of tobacco and tobacco products. RESULTS: It was determined that the use of cigarette and hookah was high in males, and it increased as the age increased in both genders (p<0.05). It was also determined that parents and friends using cigarette and hookah affected the use of cigarette and hookah in the study groups in both surveys (p<0.05). Additionally, it was determined that the educational level of the parents and being of a high social class were the social determiners of hookah use (p<0.05). CONCLUSION: It is important that we fight against the use of all tobacco products, especially in young people. Education about the damages of tobacco and applying tobacco products should start at an early age, and accurate tobacco control models may provide support in this field. © 2020 by Turkish Thoracic Society.Item Significant changes in trans-epithelial barrier proteins of adenoid tissue with atopic status in children(AVES, 2020) Yılmaz Ö.; Şimşek Y.; İnan S.; Buga Ö.; Eskiizmir G.; Pınar E.; Kanık E.; Yüksel H.OBJECTIVES: Adenoid tissue is important in local immune response and epithelial barrier dysfunction of this tissue may contribute to allergies. The aim of this study was to evaluate the relationship between the status of cross-epithelial barrier elements in adenoid tissue lymphoepithelium and inhalant allergen sensitization. MATERIALS AND METHODS: Children aged 5-15 years, who underwent adenotonsillectomy, participated in this study. All subjects underwent skin prick testing with environmental inhalant allergens. Occludin, ZO1, e-cadherin, β-catenin, desmoglein, desmoplakin, and connexon-43 were stained immunohistochemically in the adenoid tissues obtained and scored by H-score. RESULTS: We enrolled 76 children, 14 among whom were sensitized to environmental allergens. Among the zonula occludens proteins, median H-scores for occludin, claudin, and ZO-1 were significantly lower in the atopic compared to the nonatopic group respectively (p<0.001). Similarly, median H-scores for e-cadherin and β catenin proteins of the zonula adherens were significantly lower in the atopic group (p<0.001). Both desmoglein and desmoplakin H-scores were significantly lower in the atopic group [60 (50-100) vs 280 (260300), p<0.001 and 105 (87.5-120) vs 280 (67.25-300), p<0.001 respectively]. Moreover, connexin-43 protein of the gap junction was significantly lower in the atopic group (p<0.001). CONCLUSION: Adenoid tissue, which is the initial point of contact of inhalant allergens demonstrates epithelial barrier junctional protein, changes in children with inhalant allergen sensitization without clinical allergic disease symptoms. Therefore, it may be concluded that epithelial barrier function plays an important role in the development of allergen sensitization versus tolerance. © 2020 by Turkish Thoracic Society.Item Clinical outcomes of salvage revision surgery following finger replantation with vascular insufficiency: A retrospective study(AVES, 2020) Güntürk O.B.; Kayalar M.; Bali U.; Özaksar K.; Toros T.; Gürbüz Y.Objective: The aim of this study was to analyze the outcomes of revision surgery following replantation of single digital amputations. Methods: In this study, first, a total of 403 patients (339 male, 64 female mean age=28 years; age range=1-76) in whom a single finger replantation was performed were retrospectively reviewed, and then 60 patients with arterial or venous insufficiency in whom revision surgery was performed were reanalyzed. The second finger was observed to be the most injured one (32.8%). Injury type was classified as clean cut (25.3%), local crush (38.7), extensive crush (7.9%), and avulsion (28.1%). When taking the levels of injuries of the artery-only finger replantations into account, one finger (0.8%) was nail distal third, 70 fingers (56%) were nail distal third to lunula, 43 fingers (34.4%) were lunula to distal phalanx basis, 10 fingers (8%) were distal interphalangeal (DIP) joint, and one finger (0.8%) was middle phalanx. Operative revision was performed on 60 (14.9%) fingers. The need for operative revision was arterial insufficiency in 37 fingers (61.7%) and venous insufficiency in 23 fingers (38.3%). The average revision time was 43 (range=6-144) hours. While the average elapsed time for artery procedures was 35.3 (range=8-110) hours, the average elapsed time for vein procedures was 47.1 (range=6-144) hours. Finger survival rates were examined. Injury mechanism, amputation level, the number of artery/vein repairs and methods were examined in all patients and revision patients separately. Results: After the replantations, according to survival analysis, while 342 (84.9%) fingers were operated upon successfully, 61 (15.1%) fingers developed necrosis. In the patients with revision surgery, the survival rate was 78.3%. The need for revision was arterial insufficiency in 37 fingers (61.7%) and venous insufficiency in 23 fingers (38.3%). The revision rate was significantly lower than other injury types in clean-cut cases. In terms of levels of injury, no revisions were required from distal to lunula level, and the highest revision rate was observed at the proximal interphalangeal (PIP) joint level. Conclusion: The results of the present study have shown that early re-exploration can provide a 78.3% success rate and can increase the survival rate from 67.6% to 84.2% following replantation of single digital amputations. Surgical re-exploration seems to be a reasonable salvage for replanted fingers with vascular insufficiency. © 2020, AVES. Pandemic caused by SARS-CoV-2, proved to be no exception. Many healthcare workers died in the early stages of pandemic due to inadequate precautions and insufficient protection. It is essential to protect and maintain the safety of healthcare personnel for the confinement of pandemic as well as continuity of qualified healthcare services which is already under strain. Educating healthcare personnel on appropiate use of personal protective equipment (PPE) is as essential as procuring them. Material and Methods: A survey is conducted on 4927 healthcare personnel working solely with pediatric patients from 32 different centers. Education given on PPE usage were questioned and analyzed depending on age, sex, occupation and region. Results: Among four thousand nine hundred twelve healthcare personnel from 32 different centers 91% (n= 4457) received education on PPE usage. Of those who received education only 36% was given both theoretical and applied education. Although there was no differences among different occupation groups, receiving education depended on regions. Conclusion: It is essential to educate healthcare personnel appropiately nationwidely for the continuity of qualified healthcare services during the pandemic. © 2020, AVES. Novy-MacNeal-Nicolle (NNN) medium is mostly used for culture all over the world. The use of molecular methods for diagnostic purposes led to the questioning of the sensitivity of NNN medium. In this study, it is aimed to compare the performance of a new culture medium with those of NNN medium and the molecular method. Methods: Samples of 22 patients with suspected cutaneous leishmaniasis (CL) and 4 patients with suspected visceral leishmaniasis (VL) were sent to Manisa Celal Bayar University Faculty of Medicine Department of Medical Parasitology for diagnosis or confirmation from Kilis, Osmaniye, Bitlis, Gazian-tep and Manisa provinces. Pre-prepared media were sent to the health centers in these provinces and inoculation of these media with clinical samples was requested. Needle aspiration fluid for CL and bone marrow samples for VL were cultured simultaneously on NNN medium and newly developed liver extracted medium (LEM), and were sent to our institution with their smears. Smears were examined by staining with Giemsa method. In addition, parasite DNA was identified with real-time polymerase chain reaction method in these materials. Results: Leishmania amastigotes were detected in 14 of 26 samples. Promastigote proliferation was observed in 8 and 24 of the samples cultured in NNN and LEM respectively. No proliferation was detected by either method in samples of 2 patients with suspected CL. L. tropica, L. major and L. infantum/donovani were detected among isolates showing dermotropic location, and L. tropica and L. infantum/don-ovani were detected among isolates with viscerotropic location by genotyping. Conclusions: NNN medium, which is widely used in leishmaniasis di-agnosis, is not sensitive enough for the diagnosis of some Leishmania strains in Turkey. Therefore, an enrichment medium such as liver extract medium which we have presented in this study should be used for di-agnosis. However our results should be confirmed with more studies. Globally, the number of confirmed cases and deaths reached 9,653,048 and 491,128 respectively, as reported by 216 countries by June 27, 2020. Turkey had 1,396 new cases, 194,511 total cases, and 5,065 deaths by the same date. From the first case until today, the Turkish Thoracic Society (TTS) has been very proactive in educating doctors, increasing public awareness, undertaking academic studies, and assisting with public health policies. In the present report, social, academic, and management perspectives of the pandemic are presented under appropriate subtitles. During this critical public health crisis, TTS has once again demonstrated its readiness and constructive stance by supporting public health, healthcare workers, and the environment. This review summarizes the perspective of TTS on each aspect of the COVID-19 pandemic and casts light on its contributions. © 2020 by Turkish Thoracic Society.Item The effectiveness of medical prophylactic treatment on vestibular migraine and its effect on the quality of life(AVES, 2020) Çelik O.; Toker G.T.; Eskiizmir G.; İncesulu A.; Süyür N.Ş.OBJECTIVES: The aim of the present study was to determine the efficacy of propranolol treatment in patients with vestibular migraine by the Visual Analog Scale, Dizziness Handicap Inventory (DHI), Vertigo Symptom Scale, and Vestibular Disorders Activities of Daily Living Scale (VADL) and its effect on the quality of life. MATERIALS and METHODS: The study population consisted of 38 patients with vertigo/dizziness who underwent routine evaluation and ves-tibular examinations, were diagnosed with definitive vestibular migraine, and received the same medical treatment protocol (propranolol). The questionnaires and scales that were applied to the patients before and after treatment were evaluated. The results were evaluated with 95% confidence interval, and p<0.05 was accepted as statistically significant. RESULTS: The mean age of the patients was 47.55 (18-75) years, and 27 (71%) patients were female, and 11 (29%) were male. The mean total scores of the DHI before and after treatment were 50.21±22.39 (range: 8-92) and 9.31±9.86 (range: 0-58), respectively (p<0.001). The degree of disability after treatment was low in all patients (p<0.001). The total scores of the VADL before and after treatment were 186.63±79.65 (range: 32-280) and 55.52±51.89 (range: 28-273), respectively (p<0.001). There was no correlation between these two scales (p=0.235). CONCLUSION: To our knowledge, this is the first study to evaluate both the efficacy of propranolol treatment and its effects on the quality of life in vestibular migraine. The severity, frequency, and number of attacks and disability scores were reduced, and the quality of life was improved in patients with vestibular migraine with propranolol treatment. © 2020, AVES.
