Item Clinical features of patients with chronic urticaria and the results of the skin prick tests(Bilimsel Tip Yayinevi, 2019) Bilgir F.; Özdemir R.B.; Değirmenci P.; Dede B.; Kirmaz C.Objective: Chronic urticaria (CU), which is mostly idiopathic, may also be a sign of an allergic or systemic disease. A variety of diseases and allergens that accompany CU have been reported in various studies. The aim of this study was to investigate the factors that play a role in the etiology of CU. Materials and Methods: The demographic and clinical data from 302 patients – aged over 18 years - who were being monitored for CU were recorded. Several tests were performed to analyze comorbidities such as infection, malignancy, and autoimmune and rheumatic diseases. Skin Prick Test (SPT) results and questionnaires were reviewed. Results: Two hundred and thirteen out of the 302 patients with a mean age of 40.26±14.3 years were females. The duration of CU was 43.30±62.60 months and 42.9% of the patients had comorbidities. The most common disease was autoimmune thyroiditis (13.9%). Fifty-five percent of the patients were sensitive to inhalant allergens, and 5% exhibited allergen positivity for food. The most common allergens were house (43.7%) dust mites. Allergen positivity was 37.2% in those with comorbidities, while it was 62.8% in those without any comorbidities and this relationship was statistically significant (p<0.05). Conclusion: This study showed that there was an association between house dust mite and CU dust sensitivity. House dust mite sensitivity may trigger CU. © 2019 Bilimsel Tip Yayinevi. All rights reserved.Item Comparison of prolene and progrip meshes in inguinal hernia repair in terms of post-operative pain, limitation of movement and quality of life(Bilimsel Tip Yayinevi, 2020) Kaya A.; Şahin S.T.; Kaya Y.; Coşkun T.; Sakarya A.Objective: The study aimed to compare the techniques applying prolene mesh and progrip-self fixating mesh in terms of post-operative pain, limitation of movement and quality of life. Material and Methods: The study was conducted from November 2014 to January 2016 in Department of Surgery, Manisa Celal Bayar University Hospital. The study recruited 50 male patients, aged 18 and over and was carried out as a double blinded procedure. Twenty-five patients were randomly selected to receive hernia repair by progrip self-fixating mesh and 25 patients were treated with hernia repair with suture fixation method by using prolene grafts, and patients' pain follow-up was performed with face-to-face or telephone interviews with VAS (Visual Analogue Scale) and return to daily routine activities were evaluated with SF-36 (Short Form-36) quality of life scale. Recurrent hernias and emergency cases were excluded. Results: The pain scores were lower and a statistically significant difference was achieved in patients in whom progrip self-fixating mesh was used in the early postoperative period. Both methods gave statistically similar results in terms of pain and quality of life. Conclusion: In the literature, there are some evidence that the repair applied with progrip self-fixating graft has more positive outcomes compared to the repairs applied with suture fixation. It is concluded that there is a need for longer follow-ups and larger series of cases in order to achieve a definite result. © Copyright 2020 by Turkish Surgical Society.Item In-Vitro Evaluation of Immunomodulation Effects of Mesenchymal Stem Cell-Derived Exosomes in Refractory Chronic Spontaneous Urticaria(Bilimsel Tip Yayinevi, 2023) Ozdemir A.T.; Kirmaz C.; Ozdemir R.B.O.; Oztatlici M.; Sonmez P.K.; Tuglu M.I.Objective: Approximately half of chronic spontaneous urticaria (CSU) patients are thought to have an autoimmune pathology, and they are resistant to current treatment approaches. Mesenchymal stem cells (MSCs) are adult cells that have been shown to be useful in many autoimmune pathologies due to their immunomodulation properties. This study aimed to investigate the immunomodulatory effects of MSCs, and exosomes isolated from refractory CSU patients. Materials and Methods: Peripheral blood mononuclear cells (PBMCs) were isolated from 5 refractory CSU patients and 5 healthy volunteers. The effects of MSCs isolated from CSU patients and healthy MSCs were compared. Co-culture experiments were performed to evaluate the efficacy of Mesenchymal Stem Cells and exosomes on PBMCs of CSU patients and healthy volunteers. To compare the resulting effects, changes in IFN-γ, IL-4, IL-10, IL-17a, and TGF-β cytokines were detected by the ELISA method. Cell proliferations were detected with the CCK-8 kit. Results: The effects of autologous and allogeneic MSCs on IFN- γ expressions were similar, both providing significant suppression at all cell ratios. However, IL-4 and IL-10 expression of PBMCs co-cultured with allogeneic MSCs significantly decreased while IL-17a and TGF-β expression increased significantly. In addition, our findings indicated that exosomes were capable of significant suppression at low PBMC ratios, regardless of autologous or allogeneic origin, but MSCs were more effective as the number of PBMCs increased. Conclusion: These preliminary findings from in-vitro experiments suggested that allogeneic MSC, or high-dose exosome administration may be a potential approach for treatment in CSU patients, most of whom are regarded as suffering from an autoimmune disease and resistant to current treatments.
