Item Effects of a statin group drug, pravastatin, on the insulin resistance in patients with metabolic syndrome(Elsevier Masson SAS, 2004) Güçlü F.; Özmen B.; Hekimsoy Z.; Kirmaz C.Background. - In West of Scotland Coronary Prevention Study (WOSCOPS), development of type 2 diabetes mellitus (DM) was found to decrease by 30% in pravastatin-treated patients. In the study, it is suggested that pleiotropic effects of pravastatin may be responsible too as well as its lipid lowering effect. Objective. - The aim of this study was to assess the effects of pravastatin treatment on the insulin resistance in patients with metabolic syndrome with impaired glucose tolerance (IGT), by Homeostasis Model Assessment (HOMA) test, insulin sensitivity indices and glucose half activation time (glucose t1/2). Methods. - Study population consisted of 25 women who were diagnosed with metabolic syndrome. At baseline and 10 weeks after the 20 mg/daily tablet pravastatin treatment, waist/hip circumference, body weight and arterial blood pressure measurements, plasma glucose, total cholesterol, triglyceride, high density lipoprotein (HDL)-cholesterol, transaminases, glycosylated haemoglobin (A1C) and insulin level measurements were obtained along with HOMA test and insulin tolerance test after 12 h of fasting. Insulin sensitivity indices and glucose t1/2 were assessed. Results. - After the treatment, a statistically significant decrease was observed in arterial blood pressure values (P < 0.0001). While plasma total cholesterol, low density lipoprotein (LDL)-cholesterol, and triglyceride levels were found to decrease significantly and HDL-cholesterol levels increased significantly, a decrease in baseline insulin levels, an increase in insulin sensitivity levels were observed along with an decrease in glucose t1/2. Related to the improvement in aforementioned parameters, statistically significant decreases were noted in HOMA, postprandial and fasting glucose levels and A1C values (P < 0.0001). Conclusion. - Our study suggests that using pravastatin in the dyslipidemia treatment of metabolic syndrome with IGT may be an effective approach by its advantageous effects on insulin resistance. Based on this result, it is possible to say that this can be a risk lowering treatment approach for the development of type 2 DM. © 2004 Elsevier SAS. All rights reserved.Item Giant-cell tumor of the hand: Midterm results in five patients; [Tumeur à cellules géantes de la main: Résultats du traitement: 5 Cas au recul moyen de 7,8 ans](Elsevier Masson SAS, 2007) Ozalp T.; Yercan H.; Okçu G.; Ozdemir O.; Coskunol E.; Bégué T.; Calli I.Purpose of the study: Giant-cell bone tumors are benign but have great potential for recurrence. Frequently observed in epiphyseal areas of tubular bones, these tumors are rarely found in hand localizations. We examined the characteristic features of giant-cell tumors of the hand and analyzed the pertinence of surgical treatment. We noted complications, consequences of recurrence and later operations on the same tumor site in five cases. Case reports: Five patients treated between 1973 and 2000 for giant-cell tumors involving the hand bones were reviewed retrospectively. Mean age was 41.6 years and mean follow-up was 7.8 years. The surgical procedure was curettage for two, curettage with bone graft for two and amputation for one. The Enneking score was noted. Results: Pain was the main symptom, with local swelling in several cases. At 7.8 years follow-up recurrence was noted for four of the five tumors. Two patients were treated for a second recurrence. Amputation of the forearm was required for one recurrence affecting soft tissue. Mean time between two consecutive recurrences was three months. In all six episodes of recurrent tumor were treated. These five patients had a total of ten operations. There were no cases of metastasis nor multicentric foci. Discussion: Treatment of giant-cell tumors involving the hand bones is designed to eradicate the tumor and also protect hand function while keeping in mind the aggressive nature of these benign tumors. Surgical alternatives for radical treatment can include wide resection, resection of the ray and amputation. © 2007. Elsevier Masson SAS.Item Bilateral fracture of the pisiform bone; [Fracture bilatérale de l'os pisiforme](Elsevier Masson SAS, 2007) Ozalp T.; Kurt C.; Coskunol E.; Ozdemir O.; Bégué T.Fracture of the pisiform bone is exceptional compared with the other carpal bones. Such lesions may be missed in the emergency room because of the complex anatomy of the carpal region. The usual circumstances leading to fracture of the pisiform bone include fall with reception on the hyperextended wrist, traffic accidents, and blunt trauma. The purpose of this work was to present a case of bilateral fracture of the pisiform bones. A 34-year-old woman presented with pain along the ulnar borders of both wrists after a fall. The mechanism of fracture was direct trauma on both hands after reception in radial inclination. The diagnosis was established late and conservative treatment was applied. After simple immobilization, the clinical and functional outcome was excellent. The anatomic position of the pisiform bone exposes it to direct trauma. Late diagnosis can favor osteoarthritic sequelae involving the pisotriquetral joint. © 2007. Elsevier Masson SAS.Item Epidemiological profile of onychomycosis in the elderly living in the nursing homes(Elsevier Masson SAS, 2014) Gunduz T.; Gunduz K.; Degerli K.; Limoncu M.E.Introduction Few data is available about the prevalence and the causative microorganisms of onychomycosis in the elderly. The aim of this study was to determine the prevalence and types of onychomycosis in the elderly people living in the nursing homes and to determine the responsible fungal agents. Methods Elderly people living in nursing homes were examined for onychomycosis. Nails with color and structure changes were suspected of onychomycosis. Scraping materials were evaluated by direct KOH examination and fungal cultures were performed. Results Totally, 214 elderly persons (59 female, 155 male) from nursing homes were studied. Onychomycosis was suspected clinically in 102 cases and scraping materials were obtained from 122 nails. Clinical types of onyhomycosis were distal subungual in 87, proximal subungual in 21 and white superficial in 14 cases. Fungal spores were seen in 81 (37.8%) samples (3 hand nails, 78 toenails) with direct KOH examination and fungal culture was positive in 54 (25.2%) (3 hand nails, 51 toenails) of them. Trichophyton rubrum was the most frequently isolated fungus (75.9%) followed by Candida glabrata (12.9%). Conclusions In the elderly, onychomycosis is more common than the general population. However causative fungi are similar; dermatophytes are the most common causative organisms followed by yeasts. © 2013 Elsevier Masson SAS and European Union Geriatric Medicine Society.Item Forced convection of ferrofluids in a vented cavity with a rotating cylinder(Elsevier Masson SAS, 2014) Selimefendigil F.; Öztop H.F.In this study, numerical investigation of the forced convection of ferrofluid in a square cavity with ventilation ports in the presence of an adiabatic rotating cylinder is carried out. The governing equations are solved with a finite element based solver. The effects of Reynolds number (20 ≤ Re ≤ 400), angular rotational speed of the cylinder (-500 ≤ Ω ≤ 500), strength and location of the magnetic dipole (0 ≤ γ ≤ 250), (0.2 ≤ a ≤ 0.8, -0.8 ≤ b ≤ -0.2) on the flow and thermal fields are numerically studied. It is observed that the length and size of the recirculation zones can be condtrolled with magnetic dipole strength and angular rotational speed of the cylinder. When the magnetic dipole is closer to the bottom wall of the cavity, flow is accelerated towards the bottom wall with larger influence area. The increasing values of the angular rotational speed of the cylinder in the clockwise direction enhance the heat transfer. © 2014 Elsevier Masson SAS. All rights reserved.Item Coincidence of low social support and high depressive score on quality of life in elderly(Elsevier Masson SAS, 2015) Unalan D.; Gocer S.; Basturk M.; Baydur H.; Ozturk A.The aim of this study is to determine the effect of depression and social support on quality of life in elderly. Methods: This is a cross-sectional study conducted from November 2012 to January 2013. The population of the study was composed of 917 elderly people over 65 who lived at their homes in Kayseri. We used the Geriatric Depression Scale, the Multidimensional Perceived Social Support Scale, and the WHOQOL-OLD module in addition to a personal information form consisting of the demographical and sociocultural characteristics of the elderly as data collection tools. During the analysis, the correlation coefficient was examined and uni-and multivariate logistic regression analyses were applied. Results: It was found that depression and social support were significantly correlated for the total and all dimensions of the WHOQOL-OLD scale. According to the multivariate logistic regression analysis which was created from the significant variables found through univariate analysis, it was found that the higher scores from the geriatric depression scale have significant effects on the total and all other quality of life dimensions except for death and dying in a negative way; in contrast, they have a significant protective effect in the death and dying dimensions. Social support displayed a significant protective correlation in sensory abilities, autonomy, past, present and future abilities, intimacy and total scores. Conclusions: Quality of life in the elderly is affected negatively by the presence of depression while social support affects it positively. © 2015 Elsevier Masson SAS and European Union Geriatric Medicine Society.Item Effects of couple stresses on the heat transfer and entropy generation rates for a flow between parallel plates with constant heat flux(Elsevier Masson SAS, 2016) Aksoy Y.In this study, the flow and heat transfer of a fluid with couple stresses is investigated. The flow caused by the pressure gradient between parallel plates is considered as incompressible, steady and fully developed while the bottom plate is adiabatic and the upper plate is exposed to a constant heat flux. Governing equations, i.e. momentum and energy, are derived and solved analytically. Using the analytical results, effects of couple stresses on the Nusselt number as a heat transfer performance parameter and entropy generation rates in the channel are presented via graphs and tables. In addition, second law analysis is performed by calculating mean entropy generation rates and Bejan number along the channel height. © 2016 Elsevier Masson SAS. All rights reserved.Item The effect of kinetin on wheat seedlings exposed to boron(Elsevier Masson SAS, 2016) Eser A.; Aydemir T.The objective of this study was to examine relationship between boron (B) induced oxidative stress and antioxidant system in boron sensitive and tolerant wheat cultivars Bezostaya and Kutluk, and also to investigate whether Kinetin (KN) enhances the level of antioxidant system, relative growth, concentration of hydrogen peroxide (H2O2), malondialdehyde (MDA) and proline and chlorophyll content in both cultivars exposed to B stress. B treatments diminished growth and chlorophyll content whereas, it enhanced accumulation of H2O2, MDA and proline, and various antioxidant enzymes such as superoxide dismutase (SOD), catalase (CAT), ascorbate peroxidase (APX), guaiacol peroxidase (GPX) and lipoxygenase (LOX) in the shoot and root of both cultivars. However, the follow-up application of KN to the B stressed plants improved growth and chlorophyll content and further enhanced the mentioned antioxidant enzymes and level of H2O2, MDA and proline. This study thus suggests that KN improves B tolerance of the studied cultivars grown under B toxicity. © 2016Item The short-and long-Term effectiveness of transcatheter arterial embolization in patients with intractable hematuria(Elsevier Masson SAS, 2016) Korkmaz M.; Åžanal B.; Aras B.; Bozkaya H.; Çlnar C.; Güneyli S.; Gök M.; Adam G.; Düzgün F.; Oran I.Purpose: Selective transarterial embolization (TAE) of the internal iliac artery is a well-known alternative technique to control intractable bladder hemorrhage (IBH). We explored the shortand long-term effectiveness of, and clinical outcomes after, TAE in patients with IBH. Materials and methods: In this retrospective study, we reviewed the hospital records of 18 IBH patients non-responsive to conservative medications who underwent TAE between January 2003 and May 2014. The early- and long-term effectiveness of TAE was investigated in the context of hematuria control, complications, mortality, requirement for blood transfusions, and hematocrit level. Results: Sixteen of the 18 patients underwent endovascular treatment; the technical success rate was 88%. TAE allowed complete remission in 16 patients (100% clinical success). On follow-up, mean hematocrit (P = 0.003) and hemoglobin (P = 0.005) levels significantly improved. Thirteen of the 16 patients (81%) required no further emergency admission after TAE during a mean follow-up period of 18.1 months (range, 3-105 months). Conclusion: TAE is a feasible, effective, and safe technique in both the short- and long-term for the treatment of IBH. © 2015 Éditions françaises de radiologie.Item Similar effects of two different external supports on wrist joint position sense in healthy subjects: A randomized clinical trial; [Étude des effets de deux méthodes différentes de soutien externe sur la perception de la position de l'articulation du poignet chez des sujets sains : un essai clinique randomisé](Elsevier Masson SAS, 2020) Ucuzoglu M.E.; Unver B.; Sarac D.C.; Cilga G.The hand is one of the most injured organs. Proprioceptive rehabilitation decreases the incidence of injury while using external supports can increase proprioception. The aim of this study was to investigate the effects of taping and elastic bandaging on wrist joint position sense (proprioception) in healthy individuals. Sixty-eight healthy students were included in our study and randomized into two groups. External supports were to apply to the dominant hand for 24 hours in both groups. Joint position sense was evaluated with an angle reproduction test before applying the external support and 20 minutes after and then 24 hours later with the external support and after removing it. There were significant improvements in joint position sense 20 minutes after applying the external support and 24 hours later (P < 0.05). Although a significant decrease in joint position sense was observed after removing the external support compared to while wearing it (P < 0.05), there was a significant improvement in the joint position sense relative to the pre-study assessment (P < 0.05). In between group comparisons, the only significant difference was observed 20 minutes after the external support was applied: the taping group had better results in joint flexion position sense than the bandaging group (P < 0.05), but in the other assessments there were no significant differences between two groups (P > 0.05). It was found that two different types of external support can improve the wrist joint's position sense in healthy subjects. These procedures can be used as a supplemental treatment in wrist rehabilitation. © 2019 SFCM
