    Systemic mastocytosis presenting with a prominent B lymphocyte proliferation in the bone marrow and extensive fibrosis of the spleen

    Systemic mastocytosis is a disease characterized by multifocal mast cell proliferation in the bone marrow or other extracutaneous organs. Because of loosely scattered and hypo-/agranular mast cells, the diagnosis is sometimes very difficult. In the bone marrow, mast cell infiltration may be associated with prominent lymphoid infiltration leading to a misdiagnosis of a low grade non-Hodgkin lymphoma. A 49-year-old woman presented with right arm and leg pain, psychiatric symptoms, and diarrhea for four years. Physical examination and laboratory investigation revealed hepatosplenomegaly, anemia, mild thrombocytosis, mild leucocytosis and lymphocytosis. In the bone marrow biopsy, there was a prominent B lymphocyte proliferation reminiscent of a low grade non-Hodgkin lymphoma/leukemia and there were some spindle cells aggregates in paratrabecular location. The consecutive bone marrow biopsies were similar to the first. The subsequent splenectomy specimen exhibited striking fibrosis. In the lymph node sections, there was marginal zone hyperplasia. Multifocal accumulations of mast cells were strongly positive with mast cell tryptase and CD117 on immunohistochemical staining, though no metachromasia was identified in Giemsa and Toluidine Blue stained aspirates and tissue sections, probably due to hypo-/agranulation of mast cells. The case was presented to emphasize the importance of the antibody to mast cell tryptase in the diagnosis of mastocytosis and to discuss problems of differential diagnosis of systemic mastocytosis. © 2007 - IOS Press and the authors.     A case of primary pulmonary tuberculosis presenting as foreign body aspiration

    An 18-month-old child was hospitalized with presumptive diagnosis of foreign body aspiration. Chest radiography revealed atelectasis in the right lower lobe. Foreign body was not seen at rigid bronchoscopy. Tuberculin skin test was 11 mm with one scar of Bacillus Calmette-Guerin vaccine. Computerized tomography of thorax revealed conglomerating lymphadenopathy in the right hilus and calcified lymphadenopathy as well as pneumonic consolidation of the right lower lobe. Tuberculosis was diagnosed based on his tuberculin skin test and thorax computerized tomography. This case is presented to emphasize that tuberculosis should be included to the differential diagnosis of children that present with signs and symptoms of foreign body aspiration and atelectasis. © 2007 IOS Press.     Femoral cartilage thickness measurements in healthy individuals: Learning, practicing and publishing with TURK-MUSCULUS

    BACKGROUND AND OBJECTIVES: Measurement of the femoral cartilage thickness by using in-vivo musculoskeletal ultrasonography (MSUS) has been previously shown to be a valid and reliable method in previous studies; however, to our best notice, normative data has not been provided before in the healthy population. The aim of our study was to provide normative data regarding femoral cartilage thicknesses of healthy individuals with collaborative use of MSUS. METHODS: This is across-sectional study run at Physical and Rehabilitation Medicine Departments of 18 Secondary and Tertiary Centers in Turkey. 1544 healthy volunteers (aged between 25-40 years) were recruited within the collaboration of TURK-MUSCULUS (Turkish Musculoskeletal Ultrasonography Study Group). Subjects who had a body mass index value of less than 30 and who did not have signs and symptoms of any degenerative/inflammatory arthritis or other rheumatic diseases, history of knee trauma and previous knee surgery were enrolled. Ultrasonographic measurements were performed axially from the suprapatellar window by using linear probes while subjects' knees were in maximum flexion. Three (mid-point) measurements were taken from both knees (lateral condyle, intercondylar area, medial condyle). RESULTS: A total of 2876 knees (of 817 M, 621 F subjects) were taken into analysis after exclusion of inappropriate images. Mean cartilage thicknesses were significantly lower in females than males (all p< 0.001). Thickness values negatively correlated with age; negatively (females) and positively (males) correlated with smoking. Men who regularly exercised had thicker cartilage than who did not exercise (all p < 0.05). Increased age (in both sexes) and absence of exercise (males) were found to be risk factors for decreased cartilage thicknesses. CONCLUSION: Further data pertaining to other countries would be interesting to uncover whether ethnic differences also affect cartilage thickness. Collaborative use of MSUS seems to be promising in this regard. © 2014 - IOS Press and the authors. All rights reserved.

    A novel fuzzy goal programming approach with preemtive structure for optimal investment decisions

    In this article, a novel portfolio selection model is proposed. This model is essentially based on the fuzzy goal programming with preemptive structure. In order to construct this model; the portfolio risk, the return levels and the beta coefficient introduced in the Capital Asset Pricing Model are considered as the fuzzy goals, and then a preemptive priority is defined among them. In accordance with the market moving trends, the different portfolio selection models are constituted for different types of investor behaviors. This model not only takes into account the different investor strategies with respect to the market moving trends, but provides also a reasonable diversification for a portfolio in terms of the risk-return tradeoff together with beta coefficient. In the application sections, the two different periods having the upward and the downward moving trends in the Istanbul Stock Exchange National 30 Index are handled separately, then the optimal portfolios are determined using the proposed portfolio selection model accordance with different investment strategies. Finally, the optimal portfolios are compared in terms of their return performances based on the selling prices in the test periods. © 2015-IOS Press and the authors.

    Sexual function and hormone profile in young adult men with idiopathic gynecomastia: Comparison with healthy controls

    Objectives: To compare sexual function and hormone profile in male patients with gynecomastia with matched controls. MATERIALS-METHODS: Forty-seven male subjects with gynecomastia and thirty healthy controls were enrolled in this study. Serum free T3, free T4, TSH, FSH, prolactin, estradiol, total testosterone, free testosterone, DHEA-SO4, LH and total PSA were measured in the patients and controls. Sexual function of the patients and controls were evaluated using International Index of Erectile Function (IIEF). The hormone values and IIEF scores of the patients were statistically compared with the controls'. RESULTS: The mean of age, body mass index, right and left testicular volume in the patient and control group were similar. The mean FSH and free T3 values of the patients were significantly lower than the controls (p = 0:007 and p = 0:03, respectively). The mean of the other hormone values in the both groups were found to be statistically similar (p > 0:05). The mean ±SD of total IIEF scores in the patient and control group were 60:14 ± 8:78 and 65:24 ± 5:52, respectively (p = 0:007). Although the mean IIEF-erectile function, orgasmic function and intercourse satisfaction scores in the patient group were significantly lower than the control group (p < 0:001, p = 0:004 and p = 0:001, respectively), the mean IIEF-desire score of the patients was significantly higher than the controls (p = 0:002). CONCLUSION: We found that the hormone profiles (except FSH and free T3) of the patients with gynecomastia were similar with the controls.     Euler summability method of sequences of fuzzy numbers and a Tauberian theorem

    We introduce Euler summability method for sequences of fuzzy numbers and state a Tauberian theorem concerning Euler summability method, of which proof provides an alternative to that of K. Knoop[Über das Eulersche Summierungsverfahren II, Math. Z. 18 (1923)] when the sequence is of real numbers.     Tauberian theorems for statistically convergent double sequences of fuzzy numbers

    In this paper, we introduce slow oscillation and Hardy's two-sided Tauberian conditions for double sequences in n-dimensional fuzzy number space En. Besides, slow decrease and Landau's one-sided Tauberian conditions for double sequences in E1 are presented.     Acute effects of static and dynamic stretching exercises on lower extremity isokinetic strength in taekwondo athletes

    BACKGROUND: Traditional warm-up exercises generally consist of submaximal aerobic running. Thereafter static or dynamic stretching exercises appropriate to the field are advised to keep the strength, which is the main component of physical fitness, stabilized. OBJECTIVE: To examine and compare the acute effects of static stretching (SS) and dynamic stretching (DS) on the knee and ankle flexor and extensor concentric (CON) isokinetic strength in well-Trained male taekwondo athletes. METHODS: A total of 14 male taekwondo athletes who train at least 4 days a week, 90 min. a day, were tested for knee and ankle flexor and extensor isokinetic peak moment (PM) at 60 and 180â /s before and 4 min. after three different stretching exercise sessions, namely, non-stretching (NS), SS, and DS, with 48-h rest intervals in a randomized crossover study design. RESULTS: None of the exercises: NS, SS and/or DS had any effect on the concentric strength of the knee and ankle flexor and extensor muscles. CONCLUSIONS: These findings suggest that in well-Trained taekwondo athletes who are accustomed to static or dynamic movement actions may be less suscePMible to stretching-induced strength deficit. Whether this conclusion may be extended to other sporting events requires further research. ©2018-IOS Press and the authors. All rights reserved.

    On the resummation of series of fuzzy numbers via generalized Dirichlet and generalized factorial series

    We introduce semicontinuous summation methods for series of fuzzy numbers and give Tauberian conditions under which summation of a series of fuzzy numbers via generalized Dirichlet series and via generalized factorial series implies its convergence.
