Item Rational antibiotic use(Journal of Infection in Developing Countries, 2009) Tunger O.; Karakaya Y.; Cetin C.B.; Dinc G.; Borand H.Background: Development of resistance to antimicrobial agents and increase of cost as the result of unnecessary and inappropriate use of antibiotics has become a global health problem. Therefore many strategies, which are aimed at optimizing antibiotic therapy, have been developed until now. In Turkey, an antibiotic restriction policy as a governmental solution was applied to decrease the antibiotic use and especially costs by Ministry of Health in 2003. The aim of this study is to evaluate the rational antibiotic use and the impact of the implementation of new restriction policy, with their reinforcement by infectious disease specialist, on the hospital wide use of antibiotics. Methodology: The data of the inpatients received antibiotics (n=495) during January-June 2006 were compared with our previous study performed by the same methodology before the restriction policy in 1998. In both studies, prospective active daily surveillance of patients was performed by three infectious disease specialists. The appropriateness of antibiotic therapy was determined using the criteria described by Kunin and Jones. The data were analyzed by using SPSS for Windows. Results: While the rate of antibiotic use decreased from 16.6% to 11.3%, rational use increased after the restriction policy (p<0.001). Besides the specific antibiotic use increasing, prophylactic antibiotic use was found decreased (p<0.001). Mostly determined irrationality was the prophylactic uses in both studies. As expected, infectious disease specialist examinations resulted in an increase in the appropriate antibiotic use. Conclusions: The restriction policy was effective in decreasing the antibiotic consumption and increasing the rational antibiotic prescription in our hospital. Copyright © 2008 Tunger et al.Item Primary echinococcal cyst in the axillary region(Journal of Infection in Developing Countries, 2011) Ozsoy M.; Keles C.; Kahya M.; Keles G.Introduction: Human hydatid disease is a parasitic infection caused by the larval form of Echinococcus granulosus. It has worldwide distribution and is endemic in many countries, especially the Mediterranean region. It most commonly affects the liver and lungs although multi-organ involvement has been observed in 20-30% of patients. Case report: A 45-year-old woman presented to a gynaecologist because of a mass in the axillary region. Her mother and her two sisters were undergoing treatment for breast cancer. In her examination, a hard, semi-mobile, painless mass was found that was approximately 3 cm in diameter. Axillary ultrasonography showed lymphadenopathy. No abnormality was found in mammographic examination of either breast, or in abdominal ultrasonography and chest X-ray. Occult breast cancer was suspected but when the mass was excised for pathological examination the biopsy showed a hydatid cyst with germinative membranes. Subsequent lung, abdomen and brain tomography scans, whole body bone scintigraphy and hydatid serology, including indirect haemagglutination and enzyme-linked immunosorbent assay, were negative. For these reasons an isolated axillary hydatid cyst was diagnosed. Conclusion: Parasitic cysts should be considered in endemic areas in patients presenting with a soft tissue mass in the axillary region. Imaging methods should be planned to include this possibility. © 2011 Ozsoy et al.Item Pooled analysis of 1270 infective endocarditis cases in Turkey(Journal of Infection in Developing Countries, 2019) Vahabi A.; Gül F.; Garakhanova S.; Sipahi H.; Sipahi O.R.Introduction: Despite developments in medicine, infective endocarditis (IE) is still associated with significant morbidity and mortality. In this study it was aimed to systematically review the infective endocarditis literature published or presented from Turkey. Methodology: To find the published series, one national database (Ulakbim), and three international databases (Scopus, Pubmed and Sci-e) were searched between 31 October-3 November 2014. also, abstracts of congresses by three national congresses were searched for studies regarding infective endocarditis. Results: Data for 1270 patients (38.3% female, mean age 46.2, 28% prosthetic valve endocarditis) with a diagnosis of infective endocarditis were obtained from 21 reports (18 published articles and three congress abstracts). Of the 18 articles, four were in peer-reviewed medical journals indexed in national databases and 14 were in international databases. There was an underlying heart disease in 51.9% and history of dental procedure was 6.7%. Fever, heart murmur and fatigue were present in 94%, 71.4% and 69% respectively. most commonly involved site was mitral valve (43.3%), followed by aortic (33.8%) and tricuspid valve (6.4%). Staphylococcus aureus, coagulase-negative staphylococci and enterococci comprised the 22.8%, 9.7% and 7.5% of the cases while 31.1% were culture-negative. Overall mortality was 23.4%. When we compared series related to years 2008 and before and 2009 and after, the mortality rates were (24.1%-224/931) vs (20.1%-32/159), respectively (p = 0,31). Conclusion: Infective endocarditis is still associated with significant mortality. S. aureus seems to be the most common etiologic agent. One of the reasons for the high prevalence may be due to ingesting oocyst disseminated by stray cats' feces. In Turkey, most of the citizens are closely associated with stray cats and they love to pet and feed them on the streets. In this study, we aimed to determine the prevalence of T. gondii DNA in feces of stray cats living in Izmir, Turkey in order to identify the transmission potential to humans and other animals. Methodology: Feces and blood samples of 465 stray cats were investigated for the presence of T. gondii oocysts by microscopy and for the presence of T. gondii DNA by two real time PCR methods. Furthermore, serum samples were analyzed for anti-T. gondii IgG antibodies using an ELISA. Results: Oocysts were detected in 0.43% of the stray cats by microscopy. T. gondii DNA was detected in 14.37% of the stray cats' feces samples. The seroprevalence rate was 37.84%. In the feces and/or blood PCR positive group, 35.89% of them were seropositive. Among the 176 seropositive cats, T. gondii DNA was detected in feces of 27 cats (15.34%). Conclusions: This study first time showed the inter relation of T. gondii DNA in feces and blood samples and seropositivity. In sum, over 14% of the stray cats living outdoor may have an important role in transmission of toxoplasmosis to humans in Izmir as well as to other animals. © 2021 Journal of Infection in Developing Countries.
