Item Use of DNA hybridization test for diagnosing bacterial vaginosis in women with symptoms suggestive of infection(2006) Gazi H.; Degerli K.; Kurt O.; Teker A.; Uyar Y.; Caglar H.; Kurutepe S.; Surucuoglu S.The purpose of this study was to evaluate a DNA hybridization test (Affirm VPIII) as an alternative to Gram stain for the rapid diagnosis of bacterial vaginosis in women with clinical signs of vaginal infection. Vaginal specimens were collected from 321 symptomatic women, and analyzed for bacterial vaginosis by both Gram stain using Nugent criteria and DNA hybridization test. Sensitivity, specificity, positive predictive value, and negative predictive value of the DNA hybridization test were determined using the Gram staining as the standard for diagnosis of bacterial vaginosis. Of the 321 patients, 115 (35.8%) were Gram positive for bacterial vaginosis and 126 (39.2%) were negative. 80 patients (25.0%) demonstrated intermediate Gram staining that was also considered negative. The Affirm system detected G. vaginalis in 107 (93.0%) of 115 vaginal specimens positive for bacterial vaginosis diagnosed by Gram stain. Compared to the Gram stain, DNA hybridization test had a sensitivity of 87.7% and a specificity of 96.0%. Positive and negative predictive values of the DNA hybridization test were 93.0% and 92.7%, respectively. In conclusion, Affirm VPIII hybridization test correlated well with Gram stain and may be used as a rapid diagnostic tool to exclude bacterial vaginosis in women with genital complaints. Copyright © Apmis 2006.Item Does vaginal douching affect the risk of vaginal infections in pregnant women?(2006) Sakru N.; Inceboz T.; Inceboz U.; Zeren I.; Karakus M.; Kirca U.Objective: To evaluate the relationship between vaginal douching and vaginal infections among women in early pregnancy. Methods: We conducted this study in the Department of Gynecology and Obstetrics, Family Planning Center, Dr. E. Hayri Ustundag Hospital, Izmir, Turkey, between March 2003 and December 2004. We examined the vaginal swabs of 129 women, asking for termination of pregnancy in a family-planning center as both wet-preparations and cultures for vaginal microorganisms, and recorded the informations on women's vaginal douching habit. Results: Among 129 women examined, 80 had at least one type of vaginal microorganisms. Of 67 vaginal douche users, 48 (71.6%) had at least one type of vaginal organisms, whereas of 62 nonusers, only 32 (51.6%) had microorganism, although age, educational status, coital frequency, age at the first intercourse were not statistically different between the vaginal douche-users and non-users. Especially, Group B Streptococcus (GBS), Enterococcus spp. and Candida spp. were found more frequent in vaginal douche-user women. Conclusion: We found that vaginal douching tends pregnant women to genital tract the incidence of vaginal infections, especially those caused by Enterococcus spp. and GBS. As such infections may render such women to high risk in terms of perinatal mortality and morbidity, it would be appropriate to discourage vaginal douching in pregnant women.Item Vaginal douching practice: Frequency, associated factors and relationship with vulvovaginal symptoms(Pakistan Medical Association, 2016) Yanikkerem E.; Yasayan A.Objective: To determine the frequency, associated factors and relationship with vulvovaginal symptoms and vaginal douching among Turkish women. Methods: The cross-sectional, analytical study was conducted at Merkez Efendi Hospital, Manisa, Turkey, from January to June 2014 using a questionnaire. Statistical analysis was carried out using SPSS 17. Results: Of the total 343 women in the study, 91(26.5%) had reported vaginal douching in the preceding year. Statistically significant relationship was determined between the vaginal douching behaviour and couples who had low educationand low income levels, having unplanned pregnancy and had someone in their neighbourhood who douched (p<0.05 each). The most common reason for using vaginal douching was reported to be cleanliness by 85(93.4%) women, prevention of genital infections 75(82.4%), cleaning after/before sexual intercourse 72(79%), during menstruation 49(54%), prevention of vaginal discharge 69(76%), decreasing of unpleasant odours 65(71.4%) and religious beliefs 46(50.5%). Self-reported history of vaginal infection was significantly more common for women who douched compared those who did not (p<0.05). Conclusion: Healthcare providers should determine the reason and risky groups of women and educate the women to stop the vaginal douching behaviour and harmful effects of vaginal douching. © 2016, Pakistan Medical Association. All rights reserved.Item Comparison of Diagnostic Methods for Detection of Trichomonas vaginalis in Prediagnosed Vaginitis Cases and Its Association with Various Pathogens; [Vajinit Ön Tanılı Olgularda Trichomonas vaginalis Saptanmasında Tanı Yöntemlerin Karşılaştırılması ve Çeşitli Patojenlerle Birlikteliği](Galenos Publishing House, 2022) Faraşat V.T.; Balcıoğlu İ.C.; Hasdemir P.S.; Gümüş E.Objective: Parasitological diagnostic methods such as direct microscopy, staining examination and culture methods are frequently used in the diagnosis of Trichomonas vaginalis (T. vaginalis). Though, nowadays, new diagnostic methods, especially DNA-based methods, are developing, enabling the simultaneous recognition of different pathogens. In our study, we evaluated whether the choice of multiplex polymerase chain reaction (PCR), in which T. vaginalis and different pathogens can be detected, is be an alternative to classical methods and to evaluate the possible coexistence of pathogens. Methods: In our study, swab samples taken during routine examination of 100 female patients who presented to Manisa Celal Bayar University and Manisa City Hospital Outpatient Clinics Obstetrics and Gynecology were evaluated. The presence of T. vaginalis was investigated in these samples by direct microscopy, Giemsa stain and culture. Besides T. vaginalis, other possible agents were also investigated by real-time multiplex PCR method. Results: At least one agent was detected in 85 (85%) of the 100 patient samples included in our study. T. vaginalis positivity was detected in 6 (6%) of the samples by parasitological diagnosis methods and in 10 (10%) of the samples by multiplex PCR. Additionally, with real-time multiplex PCR, Chlamydia trachomatis in 4 (4%), Neisseria gonorrhoeae in 3 (3%), Ureaplasma urealyticum/parvum in 68 (68%), Gardnerella vaginalis in 68 (68%) and Herpes simplex virus 1/2 in 1 (1%) of the sample positivity was found. Mycoplasma genitalium, another agent examined by multiplex PCR, was not found positive in any sample. The Kappa value of the culture that is a parasitological test and multiplex PCR for T. vaginalis showed moderate agreement with 59.5%. Conclusion: It has been concluded that using real-time multiplex PCR method, which has high specificity and sensitivity, in addition to microscopy and culture methods in the diagnosis of T. vaginalis, could contribute to the correct and effective treatment by detecting multiple infections. © 2022, Galenos Publishing House.
