Severe hyponatremia and rhabdomyolysis associated with hypothyroidism

Abstract

Hyponatremia is one of the laboratory feature ofhypothyroidism and usually occurs in elderly patients. Inthis report we presented a young patient withhypothyroidism induced severe hyponatremia andrhabdomyolysis. A nineteen-year-old woman was broughtto the hospital with confusion. Because of acutehyponatremia with confusion, she was treated withhypertonic saline. The neurological examination returnedto normal after treatment and fluid restriction, then serumsodium level increased to normal. Because of recurrenthyponatremia futher investigations were held. She wasdiagnosed as Hashimoto's thyroiditis. Hyponatremiaimproved with levothyroxine therapy and serum sodiumlevel remained normal range. The elevated parameters ofrhabdomyolysis decreased with levothyroxine therapy.Hypothyroidism should be considered in patientspresenting with hyponatremia and rhabdomyolysis.
